心理支持联合膳食护理对淋巴瘤化疗患者营养指标的影响

摘　　要：目的探讨心理支持联合膳食护理对淋巴瘤化疗患者营养指标的影响。方法选取2022年1月至2023年12月于福建省立医院住院的280例淋巴瘤患者作为研究对象,按照随机数字表法分为研究组与对照组,每组140例(对照组失访3例,中途退出6例,对照组131例)。对照组给予常规护理干预模式,研究组给予心理支持联合膳食护理干预。比较两组营养指标[白蛋白(albumin,ALB)、前白蛋白(prealbumin,PA)、总蛋白(total protein,TP)]、营养状态评分[微型营养评估(mini nutritional assessment,MNA)]、心理状态[焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)评分]。结果干预后,两组ALB、PA、TP水平均高于治疗前,MNA评分均低于治疗前,但研究组ALB、PA、TP水平及MNA评分均高于对照组,差异有统计学意义(P<0.05)。干预后,两组SAS、SDS评分均低于干预前,且研究组低于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率为3.57%,低于对照组的12.98%,差异有统计学意义(P<0.05)。结论心理支持联合膳食护理干预淋巴瘤化疗患者效果显著,可改善患者的心理状态,提高营养水平,且安全性较高,值得临床推广应用。Objective To investigate the effects of psychological support combined with dietary nursing on nutritional indexes of patients with lymphoma chemotherapy.Methods 280 lymphoma patients hospitalized in the Fujian Provincial Hospital from January 2022 to December 2023 were selected as the study subjects,they were divided into the study group and the control group according to the random number table method,with 140 cases in each group(3 cases in the control group were lost to follow-up,6 cases were withdrawn midway,and 131 cases in the control group).The control group received routine nursing intervention,while the study group received psychological support combined with dietary care.The nutritional indicators(albumin[ALB],prealbumin[PA],total protein[TP]),nutritional status score(mini nutritional assessment[MNA])and psychological status(self-rating anxiety scale[SAS],self-rating depression scale[SDS])were compared between the two groups.Results After intervention,the levels of ALB,PA and TP of the two groups were higher than those before treatment,and the MNA score were lower than that before treatment,but the levels of ALB,PA,TP and MNA score in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After intervention,the SAS and SDS scores of the two groups were lower than those before intervention,and the study group was lower than the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 3.57%which was lower than 12.98%in the control group,the difference was statistically significant(P<0.05).Conclusion Psychological support combined with dietary nursing intervention in patients with lymphoma chemotherapy has a significant effect,which can improve the psychological state of patients,improve the nutritional level,and has high safety,it is worthy of clinical promotion and application.
