基于“从脾论治”观察香砂六君子汤改善脾虚痰浊型颈动脉斑块临床研究

摘　　要：目的探讨香砂六君子汤对于脾虚痰浊型患者颈动脉粥样硬化斑块的影响。方法选取68例诊断为脾虚痰浊型颈动脉斑块患者,采用随机数字表法将患者分为治疗组和对照组各34例。对照组给予西医常规治疗(阿托伐他汀钙片),治疗组加用香砂六君子汤强化降脂治疗,疗程3个月。比较两组患者治疗前后血清超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、血脂水平、颈动脉斑块积分、最大斑块面积、中医证候评分及中西医临床疗效。结果治疗后,两组患者血清hs-CRP、总胆固醇(total cholesterol TC)、低密度脂蛋白胆固醇水平(low densith lipoprotein Cholesterol LDL-C)、颈动脉硬化斑块积分、最大斑块面积、中医证候评分、中西医临床疗效均明显低于治疗前(P<0.05),且观察组明显低于对照组,差异有统计学意义(P<0.05)。结论香砂六君子汤联合阿托伐他汀钙片能够明显降低脾虚痰浊型颈动脉斑块患者血脂和hs-CRP水平,改善颈动脉粥样斑块,明显改善中医临床证候。Objective To explore the effect of Xiangsha Liujunzi Decoction(香砂六君子汤)on carotid atherosclerotic plaque in patients with spleen deficiency and phlegm turbidity.Methods 68 patients diagnosed with spleen deficiency and phlegm turbidity type carotid artery plaques were selected and randomly divided into a treatment group and a control group,with 34 cases in each group,using a random number table method.The control group received conventional Western medicine treatment(Atorvastatin calcium tablets),while the treatment group received intensified lipid-lowering treatment with Xiangsha Liujunzi Decoction,with a course of 3 months.To compare the serum levels of hypersensitive C-reactive protein(hs-CRP),blood lipids,carotid plaque score,maximum plaque area,traditional Chinese medicine syndrome score,and clinical efficacy of Chinese and Western medicine before and after treatment between two groups of patients.Results After treatment,the serum hs CRP,total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),carotid artery atherosclerotic plaque score,maximum plaque area,traditional Chinese medicine syndrome score,and clinical efficacy of both groups of patients were significantly lower than before treatment(P<0.05),and the observation group was significantly lower than the control group,with statistical significance(P<0.05).Conclusion Xiangsha Liujunzi Decoction combined with Atorvastatin calcium tablet can significantly reduce the levels of blood lipid and hs-CRP in patients with carotid plaque of spleen deficiency and phlegm turbidity type,improve carotid plaque,and significantly improve the clinical syndromes of traditional Chinese medicine.
