摘  要:目的探讨系统性红斑狼疮(SLE)患儿血清25-羟基维生素D_(3)[25(OH)D_(3)]水平与淋巴细胞亚群的关系。方法选取2022年6月—2023年6月海南省妇女儿童医学中心SLE患儿52例(SLE组)和健康体检儿童50名(正常对照组)。检测所有研究对象血清25(OH)D_(3)水平和淋巴细胞亚群[CD_(3)^(+)T细胞、CD4^(+)T细胞、CD8^(+)T细胞、总B细胞、自然杀伤(NK)细胞百分比和CD4^(+)/CD8^(+)比值]。参考维生素D缺乏标准将SLE患儿分为维生素D不足组[20例,25(OH)D_(3)<50 nmol·L^(-1)]和维生素D正常组[32例,25(OH)D_(3)≥50 nmol·L^(-1)]。采用Pearson相关分析评估血清25(OH)D_(3)水平与淋巴细胞亚群的相关性。结果SLE组血清25(OH)D_(3)水平和CD4^(+)T细胞百分比、CD4^(+)/CD8^(+)比值、NK细胞百分比、总B细胞百分比均低于正常对照组(P<0.05),CD_(3)^(+)T细胞百分比和CD8^(+)T细胞百分比高于正常对照组(P<0.05)。维生素D不足组CD4^(+)T细胞百分比、NK细胞百分比、总B细胞百分比和CD4^(+)/CD8^(+)比值均低于维生素D正常组(P<0.01),CD_(3)^(+)T细胞百分比和CD8^(+)T细胞百分比高于维生素D正常组(P<0.05)。SLE患儿血清25(OH)D_(3)与CD4^(+)T细胞、CD4^(+)/CD8^(+)比值、NK细胞和总B细胞均呈正相关(r值分别为0.455、0.579、0.574、0.448,P<0.001),与CD_(3)^(+)T细胞、CD8^(+)T细胞均呈负相关(r值分别为-0.574、-0.466,P<0.001)。结论SLE患儿血清25(OH)D_(3)水平与淋巴细胞亚群有关,提示维生素D水平可能在SLE的诊治中有一定的价值。Objective To investigate the relationship between serum 25-hydroxyvitamin D_(3)[25(OH)D_(3)]level and lymphocyte subsets in children with systemic lupus erythematosus(SLE).Methods A total of 52 children with SLE(SLE group)and 50 healthy children(healthy control group)were enrolled from Hainan Women and Children's Medical Center from June 2022 to June 2023.Serum 25(OH)D_(3)level and lymphocyte subsets[CD_(3)^(+)T cells,CD4^(+)T cells,CD8^(+)T cells,total B cells,natural killer(NK)cell percentage and CD4^(+)/CD8^(+)ratio]were determined.According to the vitamin D deficiency standard,SLE children were classified into vitamin D deficiency group[20 cases,25(OH)D_(3)<50 nmol·L^(-1)]and vitamin D normal group[32 cases,25(OH)D_(3)≥50 nmol·L^(-1)].The correlation between serum 25(OH)D_(3)level and lymphocyte subsets was evaluated by Pearson correlation analysis.Results The serum 25(OH)D_(3)level,CD4^(+)T cell percentage,CD4^(+)/CD8^(+)ratio,NK cell percentage and total B cell percentage in SLE group were lower than those in healthy control group(P<0.05),and the percentages of CD_(3)^(+)T cells and CD8^(+)T cells were higher(P<0.05).The percentages of CD4^(+)T cells,NK cells,total B cells and CD4^(+)/CD8^(+)ratio in vitamin D deficiency group were lower than those in vitamin D normal group(P<0.01),and the percentages of CD_(3)^(+)T cells and CD8^(+)T cells were higher(P<0.05).Serum 25(OH)D_(3)was positively correlated with CD4^(+)T cells,CD4^(+)/CD8^(+)ratio,NK cells and total B cells in SLE children(r values were 0.455,0.579,0.574 and 0.448,P<0.001).It was negatively correlated with CD_(3)^(+)T cells and CD8^(+)T cells(r values were-0.574 and-0.466,P<0.001).Conclusions Serum 25(OH)D_(3)level is associated to lymphocyte subsets in children with SLE,suggesting that vitamin D levels may have diagnostic and therapeutic value in the diagnosis and treatment of SLE.
