缺血修饰白蛋白和心型脂肪酸结合蛋白与急性冠脉综合征相关性研究的进展

摘  要:急性冠脉综合征作为一种具有高发病率和死亡率的心血管疾病,早期诊断对于后续治疗具有至关重要的意义。然而,目前临床常用的反映心肌损伤的标志物往往仅在心肌细胞发生坏死后才显著升高。相比之下,缺血修饰白蛋白和心型脂肪酸结合蛋白在心肌损害的可逆阶段即可引起血中浓度的变化,显示出较高的诊断价值。本文旨在探讨急性冠脉综合征的发病机制,并对缺血修饰白蛋白和心型脂肪酸结合蛋白与急性冠脉综合征的相关性进行分析。研究发现,缺血修饰白蛋白在急性冠脉综合征的早期诊断、病变程度评估以及预后预测方面具有显著的临床价值,其浓度在心肌坏死发生前即有所升高,可与其他指标联合应用以实现早期诊断;而心型脂肪酸结合蛋白由于其心脏组织特异性,可在心血管事件发生早期释放入血,同样在急性冠脉综合征的早期诊断、冠状动脉病变程度评估及预后预测方面发挥重要作用。Acute coronary syndrome(ACS),as a cardiovascular disease with high morbidity and mortality,early di-agnosis is of vital significance for subsequent treatment.However,the commonly used biomarkers for myocardial injury in clinical practice often only increase significantly after myocardial cells have undergone necrosis.In contrast,ischemia-modified albumin(IMA)and heart-type fatty acid-binding protein(H-FABP)can cause changes in blood concentration at the reversible stage of myocardial damage,demonstrating high diagnostic value.This article aims to explore the path-ogenesis of ACS and analyze the correlation between IMA,H-FABP and ACS.The research found that IMA has signifi-cant clinical value in the early diagnosis,assessment of lesion severity and prognosis prediction of ACS.Its concentration increases before myocardial necrosis occurs and can be combined with other indicators for early diagnosis;while H-FABP,due to its cardiac tissue specificity,can be released into the blood at an early stage of cardiovascular events and also plays an important role in the early diagnosis,assessment of coronary artery lesion severity and prognosis predic-tion of ACS.
