摘　　要：目的探究妊娠期糖尿病(GDM)并发子痫前期(PE)患者血清纤维蛋白-3(Fibulin-3)、解整合素金属蛋白酶10(ADAM10)的表达水平以及其临床意义。方法选取2022年5月至2024年5月于本院治疗的GDM患者103例,将其中57例GDM非并发PE患者作为对照组,46例GDM并发PE患者作为研究组,并将研究组根据病情程度分为轻度(23例)、中度(15例)和重度组(8例)。血清Fibulin-3和ADAM10的表达水平使用ELISA法测定,研究组血清Fibulin-3和ADAM10表达的相关性使用Pearson分析,分析GDM患者并发PE的影响因素使用Logistic回归模型,ROC分析血清Fibulin-3和ADAM10对GDM患者并发PE的诊断效能。结果与对照组相比,研究组的FBG、TG、TC、收缩压、舒张压和ADAM10水平显著升高,Fibulin-3水平显著降低(P<0.05);血清Fibulin-3在轻度和中度组的表达水平显著高于重度组,且轻度组的表达显著高于中度组(P<0.05);血清ADAM10在中度和重度组中的表达水平显著高于轻度组,且重度组的表达显著高于中度组(P<0.05)。Pearson分析表明,研究组血清Fibulin-3和ADAM10表达水平呈负相关(r=-0.506,P<0.001)。Logistic分析显示,Fibulin-3为影响GDM患者并发PE的保护因素,收缩压、舒张压和ADAM10为影响GDM患者并发PE的危险因素(P<0.05)。ROC结果显示,血清Fibulin-3和ADAM10单独诊断GDM患者并发PE的AUC为0.820和0.814,联合二者检测诊断的AUC为0.929,显著大于二者单独诊断(Z二者联合-Fibulin-3=2.175,P=0.030;Z二者联合-ADAM10=2.294,P=0.022);血清Fibulin-3和ADAM10的最佳截断值为49.98ng/mL和2.17ng/mL。结论Fibulin-3在GDM并发PE患者血清中表达显著降低,ADAM10在GDM并发PE患者血清中表达显著升高,联合检测血清Fibulin-3和ADAM10水平对GDM患者并发PE的诊断效能良好。Objective To investigate the expression levels of serum fibulin-3 and metalloproteinase 10(ADAM10)in patients with gestational diabetes mellitus(GDM)and preeclampsia(PE)and their clinical significance.Methods A total of 103 GDM patients treated in our hospital from May,2022 to May,2024 were selected for the study.Among them,57 GDM patients without complicated PE were selected as the control group,while 46 GDM patients with complicated PE were selected as the research group.The research group was further assigned into mild(23 cases),moderate(15 cases),and severe(8 cases)groups according to the severity of the disease.The expression levels of serum fibulin-3 and ADAM10 were determined using ELISA method.The correlation between serum fibulin-3 and ADAM10 expression in the research group was analyzed using Pearson correlation method.The logistic regression model was used to analyze the influencing factors of PE in GDM patients.ROC curve was drawn to evaluate the diagnostic efficacy of serum fibulin-3 and ADAM10 for PE in GDM patients.Results Compared with the control group,the levels of FBG,TG,TC,systolic blood pressure,diastolic blood pressure,and ADAM10 in the research group were significantly higher,while the level of Fibulin-3 was significantly lower(P<0.05).The expression level of serum fibulin-3 in the mild and moderate groups was significantly higher than that in the severe group,and that in the mild group was significantly higher than that in the moderate group(P<0.05).The expression level of serum ADAM10 in the moderate and severe groups was significantly higher than that in the mild group,and that in the severe group was significantly higher than that in the moderate group(P<0.05).Pearson analysis showed that the expression levels of serum fibulin-3 and ADAM10 in the research group were negatively correlated(r=-0.506,P<0.001).Logistic analysis showed that fibulin-3 was a protective factor affecting the occurrence of PE in GDM patients,while systolic blood pressure,diastolic blood pressure,and ADAM10 were ri
