摘 要:目的探讨慢性鼻窦炎和真菌性鼻窦炎病原菌的分布特征,分析影响真菌性鼻窦炎感染的因素。方法回顾性选择我院治疗鼻窦炎患者80例,根据实验室病理学检测结果分为慢性鼻窦炎患者42例,真菌性鼻窦炎患者38例,选取时间2022年9月至2024年9月,收集患者临床资料与病原菌分布特征,Logistic回归分析慢性鼻窦炎发生的真菌感染的危险因素。结果真菌性鼻窦炎组革兰阳性菌占比明显高于慢性鼻窦炎组(P<0.05);慢性鼻窦炎组年龄>40岁、糖尿病、涕血、病程>1年、CT显示钙化灶、IgE阳性占比明显低于真菌性鼻窦炎组(P<0.05);经多因素Logistic回归分析结果显示年龄>40岁、涕血、CT显示钙化灶、IgE阳性为慢性鼻窦炎发生真菌感染的危险因素(P<0.05)。结论临床工作中应重视慢性鼻窦炎和真菌性鼻窦炎病原菌分布的差异,对于存在真菌感染高危因素的患者,应加强监测和预防措施,改善患者预后。Objective To explore the distribution characteristics of pathogenic bacteria in chronic sinusitis and fungal sinusitis,and to further analyze the factors affecting fungal sinusitis infection.Methods A retrospective selection of 80 patients with sinusitis treated in our hospital was conducted.According to laboratory pathological test results,these patients were divided into the chronic sinusitis group with 42 patients and the fungal sinusitis group with 38 patients.The enrollment period was from September,2022 to September,2024.Clinical data and pathogen distribution characteristics of patients were collected,and logistic regression analysis was conducted to identify risk factors for fungal infections in chronic sinusitis.Results The proportion of Gram-positive bacteria in the fungal sinusitis group was significantly higher than that in the chronic sinusitis group(P<0.05);For the chronic sinusitis group,the age of chronic sinusitis group>40 years old,diabetes,bloody nose,course of disease>1 year,CT showed calcification,and IgE positive rate were significantly lower than fungal sinusitis group(P<0.05);Multiple logistic regression analysis showed that age>40 years old,nasal bleeding,CT showing calcified lesions,and IgE positivity were risk factors for fungal infection in chronic sinusitis(P<0.05).Conclusion The difference in pathogen distribution between chronic sinusitis and fungal sinusitis should be fully considered in clinical practice.For patients with high-risk factors for fungal infections,monitoring and preventive measures should be strengthened to further improve patient prognosis.
