摘 要:目的探究糖尿病视网膜病变(DR)病人血清转录激活因子1(STAT1)、醌氧化还原酶1(NQO1)水平与视力损伤程度的关系。方法选取2019年1月—2023年1月我院收治的2型糖尿病病人122例,根据是否发生DR分为未发生DR组(NDR组,62例)和发生DR组(DR组,60例)。分析DR病人血清STAT1和NQO1表达水平与视力损伤程度的关系。结果与NDR组相比,DR组病程、餐后2 h血糖、空腹血糖、三酰甘油、总胆固醇、肌酐、肾小球滤过率显著升高(t=2.824~14.629,P<0.05),血清中STAT1水平显著升高(t=7.443,P<0.05),NQO1水平显著降低(t=7.648,P<0.05),视力损伤程度升高(Z=71.133,P<0.05)。DR病人血清STAT1和NQO1水平呈负相关关系(r=-0.452,P<0.001),STAT1水平与视力损伤严重程度呈正相关(r=0.621,P<0.05),NQO1水平与视力损伤严重程度呈负相关(r=-0.584,P<0.05)。由STAT1和NQO1构建的联合诊断模型诊断DR发生的受试者工作特征曲线下面积(AUC)为0.940(95%置信区间=0.898~0.981),高于STAT1和NQO1单独诊断的AUC(Z=5.625、6.043,P<0.05)。结论DR病人血清中STAT1高表达,NQO1低表达,且均与视力损伤严重程度有关,二者联合检测能够提高对DR的诊断价值。Objective To investigate the correlation of the levels of serum activator of transcription factor 1(STAT1)and quinone oxidoreductase 1(NQO1)with the severity of visual impairment in patients with diabetic retinopathy(DR).Methods A total of 122 patients with type 2 diabetes who were admitted to our hospital from January 2019 to January 2023 were enrolled,and according to the presence or absence of DR,they were divided into non-DR group(NDR group with 62 patients)and DR group with 60 patients.The correlation between the expression levels of serum STAT1 and NQO1 and the severity of visual impairment in patients with DR was analyzed.Results Compared with the NDR group,the DR group had significant increases in the course of disease,2-hour postprandial plasma glucose,fasting blood glucose,triglyceride,total cholesterol,creatinine,and glomerular filtration rate(t=2.824-14.629,P<0.05),a significant increase in the serum level of STAT1(t=7.443,P<0.05),a significant reduction in the level of NQO1(t=7.648,P<0.05),and a significant increase in the severity of visual impairment(Z=71.133,P<0.05).For the patients with DR,the expression level of serum STAT1 was negatively correlated with that of NQO1(r=-0.452,P<0.001),and the level of STAT1 was positively correlated with the severity of visual impairment(r=0.621,P<0.05),while the level of NQO1 was negatively correlated with the severity of visual impairment(r=-0.584,P<0.05).The combined diagnostic model established based on STAT1 and NQO1 had an area under the receiver operating characteristic curve(AUC)of 0.940(95%CI=0.898-0.981)in the diagnosis of DR,which was higher than the AUC of STAT1 and NQO1 alone(Z=5.625,6.043,P<0.05).Conclusion There is a high expression level of serum STAT1 and a low expression level of NQO1 in patients with DR,and both STAT1 and NQO1 are correlated with the severity of visual impairment.Combined measurement of STAT1 and NQO1 has a higher value in the diagnosis of DR.
