摘  要:目的了解四川省农村地区结核分枝杆菌的数目可变串联重复序列(MIRU-VNTR)基因多态性特征及耐药情况,为四川省有效防控结核病提供一定的分子流行病学理论依据。方法采用固体比例法对2019—2020年四川省农村地区的结核分枝杆菌进行药敏试验,以及采用MIRU-VNTR技术进行基因分型,并对结果进行聚类分析及各位点进行分辨力评价。运用SPSS 19.0软件进行χ^(2)检验或Fisher确切概率分析,以P<0.05为差异有统计学意义。结果纳入研究的222株结核分枝杆菌共分为5个基因群(Ⅰ、Ⅱ、Ⅲ、Ⅳ和Ⅴ群),Ⅰ群菌株占比最多,在单耐药、耐多药、多耐药及其他耐药类型中,Ⅰ群与其他基因群的菌株耐药率相比,差异均无统计学意义(P均>0.05);有206个基因型,其中191株是独立型菌株,31株是成簇菌株,共15簇,成簇率为13.96%,近期感染率最小估计为7.21%。北京家族菌有133株,占比为59.91%,且北京家族菌的成簇率显著高于非北京家族菌,差异有统计学意义(χ^(2)=6.450,P=0.011)。9个位点中,QUB18分辨指数最高(0.855);MIRU31和VNTR2372的多态性程度适中,分辨指数为0.575、0.545;其余6个位点的分辨指数分别为0.851、0.830、0.790、0.750、0.684和0.658。结论初步证实四川省农村地区结核分枝杆菌存在基因多态性,Ⅰ群为主要流行群,且与耐药无明显的相关性。Objective To understand MIRU-VNTR gene polymorphism and drug resistance of mycobacterium tuberculosis in rural areas of Sichuan Province,and to provide a theoretical basis of molecular epidemiology for effective prevention and control of tuberculosis in Sichuan Province.Methods The drug susceptibility test of mycobacterium tuberculosis was performed by using solid proportion method in rural areas of Sichuan Province from 2019 to 2020,and the genotyping was tested by MIRU-VNTR technology.Cluster analysis of the results and resolution evaluation of each point were conducted.SPSS 19.0 was used for χ^(2)test or Fisher exact probability analysis,and P<0.05was considered as statistically significant.Results The 222 strains of mycobacterium tuberculosis included in the study were divided into 5 gene groups(groups Ⅰ,Ⅱ,Ⅲ,Ⅳ,Ⅴ),and group Ⅰ accounted for the largest proportion of strains.There was no significant difference in drug resistance rate between groupⅠand other groups in single drug resistance,multi-drug resistance,multi-drug resistance and other drug resistance types(all P>0.05).There were 206 genotypes,including 191 independent strains and 31 cluster strains in a total of 15 clusters.The clustering rate was 13.96%,and the minimum recent infection rate was estimated to be 7.21%.Among 222 strains of mycobacterium tuberculosis,133 strains were from Beijing family,accounting for 59.91%.The clustering rate of Beijing family bacteria was significantly higher than that of non-Beijing family bacteria(χ^(2)=6.450,P=0.011).Among the 9 sites,the highest resolution index of QUB18 was0.855.The polymorphism of MIRU31 and VNTR2372 was moderate,and the resolution indices were 0.575 and 0.545,respectively.The resolution indices of the other 6 sites were 0.851,0.830,0.790,0.750,0.684 and 0.658,respectively.Conclusions It was preliminarily confirmed that there were gene polymorphisms in mycobacterium tuberculosis in rural areas of Sichuan Province,and group Ⅰ was the main epidemic group and had no obvious correlati
