摘　　要：目的 分析电子支气管镜肺泡灌洗治疗细菌性肺炎患者的临床效果。方法 78例细菌性肺炎患者随机分为两组。对照组予以氨溴索治疗,观察组在对照组基础上采用电子支气管镜肺泡灌洗治疗。比较两组的治疗效果及不良反应。结果 观察组的治疗总有效率高于对照组,发热时间、呼吸困难消失时间、肺部湿啰音消失时间、咳嗽消失时间及住院时间均短于对照组,A-PACHEⅡ评分低于对照组(P<0.05)。治疗后,观察组的RAW、WOB、PIP、PaCO_(2)、IL-6、PCT、CRP水平均低于对照组,PaO_(2)、SpO_(2)水平均高于对照组(P<0.05)。两组的不良反应发生率对比,差异无统计学意义(P>0.05)。结论 电子支气管镜肺泡灌洗治疗细菌性肺炎患者的效果显著,可加快症状消退,提高呼吸功能,改善血气指标,减轻炎症症状,且安全性较佳。Objective To explore the clinical effect of electronic bronchoscopic alveolar lavage in the treatment of patients with bacterial pneumonia.Methods 78 patients with bacterial pneumonia were randomly divided into two groups.The control group was treated with ambroxol,and the observation group was treated with electronic bronchoscopic alveolar lavage on the basis of the control group.The treatment effect and adverse reaction were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the duration of fever,disappearance time of dyspnea,disappearance time of pulmonary wet rales,disappearance time of cough,and hospitalization time were shorter than those in the control group,and APACHEⅡscore was lower than that in the control group(P<0.05).After treatment,the levels of RAW,WOB,PIP,PaCO_(2),IL-6,PCT,and CRP in the observation group were lower than those in the control group,and the levels of PaO_(2) and SpO_(2) were higher than those in the control group(P<0.05).No statistically significant difference was found in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Electronic bronchoscopic alveolar lavage has obviously effect in the treatment of patients with bacterial pneumonia,which can accelerate symptom resolution,improve respiratory function,improve blood gas indicators and alleviate inflammatory symptoms,and has good safety.
