血管内超声指导PCI治疗对冠心病患者心功能及主要心血管不良事件发生情况的影响

摘　　要：目的 探讨血管内超声(IVUS)指导经皮冠状动脉介入(PCI)治疗对冠心病患者的影响。方法 选取2022年4月至2024年4月南阳市中心医院收治的72例冠心病患者,随机分为两组各36例。参照组采用冠状动脉造影(CAG)指导PCI治疗,研究组采用IVUS指导PCI治疗。比较两组的支架植入效果、心功能、血管及斑块状况、并发症、主要心血管不良事件发生情况。结果 研究组的支架直径、支架长度、最大管腔直径、最小管腔直径均长于参照组,最大球囊压力高于参照组,残余狭窄率低于参照组(P <0.05)。术后,研究组的LVEF、 E/A及SV均高于参照组,MID长于参照组,DS、 PB均低于参照组(P <0.05)。两组的术后并发症发生率比较,差异无统计学意义(P>0.05)。术后随访6个月期间,研究组的主要心血管不良事件发生率为2.78%,低于参照组的22.22%(P <0.05)。结论 IVUS指导PCI治疗可有效提高冠心病患者的支架植入效果,促进患者心功能恢复,并可明显改善患者的血管及斑块状况,降低主要心血管不良事件发生率,且未显著增加术后并发症发生风险。Objective To explore the impact of intravascular ultrasound(IVUS)-guided percutaneous coronary intervention(PCI)treatment on patients with coronary heart disease.Methods 72 patients with coronary heart disease admitted to Nanyang Central Hospital from April 2022 to April 2024 were selected and randomly divided into two groups,with 36 cases in each group.The reference group received coronary angiography(CAG)-guided PCI treatment,while the study group received IVUS-guided PCI treatment.The stent implantation effect,cardiac function,vascular and plaque status,complications,and occurrence of major adverse cardiovascular events were compared between two groups.Results The stent diameter,stent length,maximum lumen diameter,and minimum lumen diameter of the study group were longer than those of the reference group,the maximum balloon pressure was higher than that of the reference group,and the residual stenosis rate was lower than that of the reference group(P<0.05).After surgery,the LVEF,E/A,and SV of the study group were higher than those of the reference group,the MID was longer than that of the reference group,and the DS and PB were lower than those of the reference group(P<0.05).No statistically significant difference was found in the incidence of postoperative complications between the two groups(P>0.05).During the 6-month postoperative follow-up,the incidence of major adverse cardiovascular events of the study group was 2.78%,lower than the 22.22%of the reference group(P<0.05).Conclusions IVUS-guided PCI treatment can effectively improve the stent implantation effect in patients with coronary heart disease,promote the recovery of cardiac function,obviously improve the vascular and plaque status of patients,and reduce the incidence of major adverse cardiovascular events,without significant increase of the risk of postoperative complications.
