摘 要:目的 分析肌骨超声对膝关节骨关节炎(KOS)的诊断价值。方法 选取2022年6月至2024年5月我院收治的90例疑似KOS患者,均实施肌骨超声、磁共振扫描检查,以出院综合诊断结果作为标准,比较两种检查方式的诊断效能以及对KOS组织损伤类型的检出率,并观察KOS患者的肌骨超声特征。结果 出院综合诊断结果显示KOS阳性72例,阴性18例;磁共振扫描显示阳性70例,阴性20例;肌骨超声显示阳性71例,阴性19例。肌骨超声诊断KOS的灵敏度、特异性、准确率、阳性预测值、阴性预测值与磁共振扫描比较,差异无统计学意义(P>0.05)。以出院综合诊断结果为标准,肌骨超声对退行性软骨病变、髌上囊积液、半月板损伤、滑膜增厚、韧带损伤的检出率均高于磁共振扫描(P <0.05)。结论 肌骨超声诊断KOS的效能较高,可确认膝关节骨关节炎的类型,为临床治疗提供参考。Objective To analyze the diagnostic value of musculoskeletal ultrasound for knee osteoarthritis(KOS).Methods A total of 90 patients with suspected KOS admitted to our hospital from June 2022 to May 2024 were selected.All of them were examined by musculoskeletal ultrasound and magnetic resonance scanning.With the comprehensive diagnosis at discharge as the standard,the diagnostic efficacy and the detection rates of KOS tissue damage types were compared between the two diagnostic methods,and the musculoskeletal ultrasound characteristics of patients with KOS were observed.Results The final comprehensive diagnosis at discharge showed that 72 cases were KOS positive and 18 cases were KOS negative;Magnetic resonance scanning showed 70 positive cases and 20 negative cases;Musculoskeletal ultrasound showed 71 positive cases and 19 negative cases.No statistically significant difference was found in the sensitivity,specificity,accuracy,positive predictive value,negative predictive value between musculoskeletal ultrasound and magnetic resonance scanning in the diagnosis of KOS(P>0.05).According to the final comprehensive diagnosis at discharge,the detection rates of degenerative cartilage lesions,suprapatellar bursal effusion,meniscal injury,synovial thickening and ligament injury by musculoskeletal ultrasound were higher than those by magnetic resonance scanning(P<0.05).Conclusions Musculoskeletal ultrasound in the diagnosis of KOS has high efficacy,which can accurately determine the types of knee osteoarthritis,and provide reference for clinical treatment.
