摘 要:目的 探讨多层螺旋CT (MSCT)检查在肺磨玻璃结节(GGN)良恶性鉴别诊断中的应用价值。方法 选取100例GGN患者,均进行MSCT检查。以病理诊断结果作为金标准,计算MSCT诊断GGN的效能,比较良性与恶性GGN患者的MSCT参数特征。结果 100例GGN患者中,恶性67例,良性33例。MSCT检查显示恶性63例,良性37例;MSCT诊断GGN的敏感度为92.54%、特异度为96.97%、准确率为94.00%,与病理结果的一致性极好(Kappa=0.868, P <0.001)。恶性病变GGN患者的病灶直径、平扫CT值、增强CT值均大于良性病变GGN患者,m GGN、形状不规则、边界模糊、毛刺、分叶、血管集束征、胸膜凹陷征、支气管充气征、空泡征检出率均高于良性病变GGN患者(P <0.05)。结论 MSCT检查鉴别诊断GGN良恶性的效能较高,观察MSCT参数特征可为GGN良恶性的鉴别诊断提供依据。Objective To explore the application value of multi-slice spiral CT(MSCT)examination in the differential diagnosis of benign and malignant pulmonary ground glass nodules(GGN).Methods 100 patients with GGN were examined by MSCT.With the pathological diagnosis results as the gold standard,the efficiency of MSCT in diagnosing GGN was calculated,and the MSCT parameter characteristics were compared between benign and malignant GGN patients.Results Among 100 patients with GGN,67 were malignant and 33 were benign.MSCT examination showed 63 cases of malignancy and 37 cases of benign;The sensitivity,specificity,and accuracy of MSCT in diagnosing GGN were 92.54%,96.97%,and 94.00%,respectively,with excellent consistency with pathological results(Kappa=0.868,P<0.001).The lesion diameter,plain CT value,and enhanced CT value of GGN patients with malignant lesions were greater than those of GGN patients with benign lesions,and the detection rates of mGGN,irregular shape,blurred boundary,spiciness,lobulation,vascular bundle sign,pleural indentation sign,bronchial inflation sign,and vacuolar sign were higher than those in GGN patients with benign lesions(P<0.05).Conclusions MSCT examination has high efficiency in distinguishing benign and malignant GGN,and observing the MSCT parameter characteristics can provide a basis for the differential diagnosis of benign and malignant GGN.
