基于3H优质护理理念的集束化护理在重症肺炎机械通气患儿护理中的应用效果

摘　　要：目的 探讨基于3H优质护理理念的集束化护理在重症肺炎机械通气患儿护理中的应用效果。方法 选取湛江中心人民医院儿童重症监护室(PICU)2021年1月至2023年12月收治的62例重症肺炎机械通气患儿,按随机数字表法分为对照组(31例)和观察组(31例)。对照组采取常规护理,观察组采取基于3H优质护理理念的集束化护理,比较两组症状改善时间、血气指标及并发症总发生率。结果 观察组退热、气促缓解、止咳、机械通气及PICU住院时间短于对照组,差异有统计学意义(P <0.05);护理1周后,观察组血氧饱和度、动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组,差异有统计学意义(P <0.05);观察组并发症总发生率(6.45%)低于对照组(25.81%),差异有统计学意义(P <0.05)。结论 基于3H优质护理理念的集束化护理在重症肺炎机械通气患儿护理中的应用效果较好,能促进患儿症状缓解及血气指标改善,降低并发症率。Objective To explore the application effect of bundled nursing based on the 3H high-quality nursing concept in the nursing of mechanically ventilated children with severe pneumonia.Methods A total of 62 children with severe pneumonia who underwent mechanical ventilation admitted to pediatric intensive care unit(PICU),Central People's Hospital of Zhanjiang from January 2021 to December 2023 were selected and divided into control group(31 cases) and observation group(31 cases) according to random number table method.The control group received routine nursing,and the observation group received bundled nursing based on the 3H high-quality nursing concept.The symptom improvement time,blood gas indexes and total complication rate of the two groups were compared.Results The antipyretic time,shortness of breath relief time,cough relief time,mechanical ventilation time and length of hospitalization in PICU in the observation group were shorter than those in the control group,the differences were statistically significant(P < 0.05).After one week of nursing,the oxygen saturation of blood and arterial partial pressure of oxygen in the observation group were higher than those in the control group,and arterial partial pressure of carbon dioxide was lower than that in the control group,the differences were statistically significant(P < 0.05).The total complication rate of observation group(6.45%) was lower than that of control group(25.81%),and the difference was statistically significant(P < 0.05).Conclusion The application of bundled nursing based on the 3H high-quality nursing concept in the nursing of children with severe pneumonia with mechanical ventilation is better,which can promote the relief of symptoms and the improvement of blood gas indexes,and reduce the complication rate.
