摘 要:目的探究特发性膜性肾病(IMN)患者黄腻苔和白腻苔的舌苔菌群特征。方法选择确诊为IMN的患者24例,其中黄腻苔13例组成黄腻苔组,白腻苔11例组成白腻苔组,另招募健康人10例组成健康组。收集各组舌苔菌群样本,运用16S rRNA全长测序技术对舌苔样本进行测序,对舌苔菌群Alpha多样性、Beta多样性、群落组成及差异进行微生物学信息分析,并基于MetaCyc数据库进行菌群功能预测分析。结果白腻苔组、黄腻苔组和健康组舌苔菌群之间Alpha多样性及Beta多样性差异无统计学意义(P>0.05)。与健康组比较,白腻苔组和黄腻苔组卟啉单胞菌属的相对丰度减少(P<0.05)。黄腻苔组弯曲杆菌属、弯曲杆菌_sp002139875和胎儿弯曲杆菌的相对丰度较白腻苔和健康组增高;白腻苔组生痰二氧化碳噬纤维菌的相对丰度较其他两组增高(P<0.05)。菌群功能预测结果显示,与健康组比较,白腻苔组和黄腻苔组的磺酸内酯降解超级途径、雄烯二酮降解Ⅰ(需氧)、甲苯降解通路途径下调(P<0.05);与白腻苔组比较,黄腻苔组丙酮酸发酵生成丙酸Ⅰ、琥珀酸发酵生成丁酸盐、甲苯降解途径下调,而D-葡萄糖醛酸和D-半乳糖醛酸降解超级途径上调(P<0.05)。结论IMN患者舌苔菌群失调,其中卟啉单胞菌属可能成为IMN的微生物学表征;弯曲杆菌属、弯曲杆菌_sp002139875、胎儿弯曲杆菌可能是IMN黄腻苔的标志菌;生痰二氧化碳噬纤维菌可能是IMN白腻苔的标志菌种。Objective To explore the characteristics of tongue coating microbiomes of yellow and white greasy coatings in patients with idiopathic membranous nephropathy(IMN).Methods Twenty-four patients diagnosed with IMN were selected.Thirteen patients with yellow greasy coating were assigned to the yellow greasy group,and eleven patients with white greasy coating were assigned to the white greasy group.Additionally,ten healthy individuals were recruited as a control group.Tongue coating microbiome samples were collected from each group and analyzed using 16S rRNA full-length sequencing.The microbial diversity(Alpha and Beta diversity),community composition,and differences were analyzed.Furthermore,functional prediction of the microbiomes was performed based on the MetaCyc database.Results There was no significant difference in Alpha and Beta diversity among the tongue coating microbiomes of the yellow greasy group,white greasy group,and healthy group(P>0.05).Compared to the healthy group,the relative abundance of Porphyromonas species was reduced in both the white greasy and yellow greasy groups(P<0.05).The yellow greasy group showed an increased relative abundance of Campylobacter species,Campylobacter_sp002139875,and Fetal Campylobacter,compared to the white greasy and healthy groups.The white greasy group had a higher relative abundance of Fibrobacteres species,which are involved in phlegm production,than the other two groups(P<0.05).Functional prediction analysis revealed that,compared to the healthy group,the white greasy and yellow greasy groups showed downregulation in the pathways of sulfonate ester degradation,androstenedione degradationⅠ(aerobic),and toluene degradation(P<0.05).In comparison to the white greasy group,the yellow greasy group showed downregulation in the pathways of pyruvate fermentation to propionateⅠ,succinate fermentation to butyrate,and toluene degradation,while upregulating the superpathway of D-glucuronic acid and D-galacturonic acid degradation(P<0.05).Conclusion The tongue coating micr
