摘  要:目的了解2018—2021年无锡市人偏肺病毒(hMPV)的流行特征及F基因变异情况,为无锡市hMPV的防控提供参考依据。方法采集2018—2021年无锡市两家哨点医院的急性呼吸道感染样本,采用实时荧光定量RT-PCR方法对采集的样本进行hMPV核酸检测,阳性样本进行F基因测序,使用Bioedit 7.0软件进行多序列比对,MEGA 7.0软件构建系统发生树,与相应参考株比对分析,确定基因型别及F基因变异情况。结果2018—2021年hMPV总检出率为7.43%(616/8292),不同年份检出率差异有统计学意义(χ^(2)=84.312,P<0.05)。3~5岁儿童检出率最高为9.71%(117/1205),冬春季为主要的流行高峰。经F基因测序,2018—2021年无锡市hMPV有3种亚型(A2b、B1、B2),主要以A2b亚型为主,B1亚型主要集中在2018年1~4月,B2亚型在2020年12月至2021年2月期间成为优势流行株。31株A2b流行株与参考株(NCBI登录号:GQ153651)比较,核苷酸和氨基酸同源性很高(核苷酸同源性:96.20%~99.50%,氨基酸同源性:97.90%~100%),仅3株流行株出现各1个突变位点;10株B1流行株与参考株(AY525843)比较,均有16个核苷酸突变位点,均仅有1个氨基酸突变位点(W396R);19株B2流行株与参考株(DQ843658)比较,均有10个核苷酸突变位点,B2流行株分为2个进化亚支(Ⅰ、Ⅱ),Ⅰ亚支比Ⅱ亚支多4个核苷酸突变位点,均有3个氨基酸突变位点(D280 N、I392 T、R396Q)。结论2018—2021年无锡市hMPV的易感人群为3~5岁的儿童,冬春季为主要流行高峰,以A2b亚型流行为主,3种亚型与参考株比较氨基酸同源性高,遗传变异较小。Objective To understand the epidemiological characteristics and F gene variation of human metapneumovirus(hMPV)in Wuxi city from 2018-2021,so as to provide the reference basis for the prevention and control of hMPV in Wuxi city.MethodsAcute respiratory infection samples were collected from two sentinel hospital in Wuxi from 2018-2021 and were tested by real-time fluorescence quantitative RT-PCR for the nucleic acids of hMPV.The positive samples were subjected to F gene sequencing.The Bioedit 7.0 software was used for multiple sequence comparison and MEGA 7.0 software was used to construct the phylogenetic tree to compare with the reference stains to determine the genotypes and F gene variants.ResultsThe total detection rate of hMPV in 2018-2021 was 7.43%(616/8292),and the differences in the detection rates of the years were statistically significant(χ^(2)=84.312,P<0.05).The detection rate was 9.71%(117/1205)in children aged 3~5 years old and was the hightest.The epidemic peaks occurred in winter and spring.The F gene sequencing revealed that there were three hMPV subtypes(A2b,B1,B2)in Wuxi city in 2018-2021 and the dominant subtype was A2b.The B1 subtype was observed mainly from January to April,2018.The B2 subtype became the prevalent type from December 2020 to February 2021.For the comparison of 31 prevalent strains of A2b with the reference strain(NCBI Accession:GQ153651),the nucleotide and amino acid similarities were high(nucleotide similarity:96.20%~99.50%,amino acid similarity:97.90%~100%).There was 1 mutation site in each of 3 epidemic strains only.Comparing to the reference stains(AY525843),all 10 epidemic strains of B1 subtype had 16 nucleotide mutation sites and only 1 amino acid mutation site(W396R).All 19 epidemic strains of B2 had10 nucleotide mutation sites comparing to the reference strain(DQ843658).The epidemic strains of B2 subtype were divided into 2 evolutionary sub-branches(Ⅰ、Ⅱ).The sub-branch Ⅰ had 4 more nucleotide mutation sites than the sub-branch Ⅱ and had 3 amino acid mutatio
