摘 要:目的观察八珍汤对腹腔镜胃癌根治术后患者胃肠功能、营养状况的影响。方法将2022年1月至2024年5月收治的胃癌术后患者分为两组。八珍汤组术后给予八珍汤治疗,安慰组术后给予安慰剂治疗。比较两组一般情况、不良反应、中医症状评分、胃肠激素、营养指标、应激反应、Shannon指数、Chaol指数、肠道菌群的差异。结果八珍汤组术后肠鸣音恢复时间、首次肛门排气时间、排便时间及住院时间短于安慰组,差异有统计学意义(P<0.05)。两组术后14d时胃肠激素、过氧化氢酶(CAT)、营养指标、超氧化物歧化酶(SOD)均高于术后1d,且八珍汤组更高(P<0.05)。两组术后14d时中医症状评分及胰高血糖素(GLU)、丙二醛(MDA)均低于术后1d,且八珍汤组更低(P<0.05)。两组术后14d时Shannon指数、Chaol指数、双歧杆菌、乳酸杆菌均高于术后1d,且八珍汤组更高(P<0.05)。两组术后14d时葡萄球菌、大肠杆菌均低于术后1d,且八珍汤组更低(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论八珍汤可促进腹腔镜胃癌根治术患者胃肠功能和营养指标的恢复,可能与减轻应激反应,调节胃肠激素、肠道菌群有关。Objective To observe the effect of Bazhen Decoction(BZD)on gastrointestinal function and nutritional status of patients af⁃ter laparoscopic radical surgery for gastric cancer.Methods The postoperative gastric cancer patients admitted from January 2022 to May 2024 were divided into two groups.The BZD group were treated with BZD postoperatively,while the placebo group was given placebo treatment.Differences between the two groups were compared in terms of general conditions,adverse reactions,TCM symptom scores,gastrointestinal hormones,nutritional indicators,stress responses,Shannon index,Chao1 index,and intestinal flora.Results The post⁃operative bowel sound recovery time,first flatus time,defecation time and hospitalization time of the BZD group were shorter than those of the placebo group,and the differences were statistically significant(P<0.05).On postoperative day 14,gastrointestinal hormones,catalase(CAT),nutritional indexes,and superoxide dismutase(SOD)were higher in both groups than those on day 1,with a signifi⁃cant increase in the BZD group(P<0.05).TCM symptom scores,glucagon(GLU)and malondialdehyde(MDA)on postoperative day 14 were lower in both groups than those on day 1,with a significant decrease in the BZD group(P<0.05).Shannon index,Chao1 in⁃dex,bifidobacterium and lactobacillus on postoperative day 14 were higher than those on postoperative day 1 in both groups,and signifi⁃cantly higher in the BZD group(P<0.05).Staphylococcus and Escherichia coli on postoperative day 14 were lower in both groups than those on postoperative day 1,and the BZD group was at significantly lower levels(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion BZD can promote the recovery of gastrointestinal func⁃tion and nutritional indexes in patients undergoing laparoscopic radical gastrectomy,which may be related to reducing the stress response,and regulating the gastrointestinal hormones and intestinal flora.
