高血压合并冠心病病人的舌象特征研究

摘　　要：目的:分析高血压及高血压合并冠心病病人的舌象特征,结合实验室指标等为临床高血压合并冠心病病人的辨证论治提供参考依据。方法:选取2023年3月—2023年7月安徽中医药大学第一附属医院心内科住院病人101例,分为单纯高血压组65例,高血压合并冠心病组36例,同期社区体检中心健康体检者50名作为健康对照组。使用“中医舌诊AI开放平台”采集舌象、判读舌象、提取HSV参数。结果:HSV参数方面,健康对照组舌质尖部、中部、边部的H值均高于单纯高血压组、高血压合并冠心病组(P<0.05或P<0.01),舌质所有部位(包括舌尖、舌中、舌根、舌边)的S值均低于单纯高血压组、高血压合并冠心病组(P<0.05或P<0.01)。高血压合并冠心病组舌质尖部、中部饱和度S值均高于单纯高血压组(P<0.05或P<0.01)。健康对照组舌苔中部、根部H值均高于单纯高血压组、高血压合并冠心病组(P<0.05或P<0.01),舌苔所有部位S值均低于单纯高血压组、高血压合并冠心病组(P<0.05或P<0.01)。高血压合并冠心病组舌苔中部、根部H值均低于单纯高血压组(P<0.05或P<0.01),舌苔尖部、根部、边部S值均高于单纯高血压组(P<0.05或P<0.01)。舌色方面,高血压合并冠心病病人以红/绛舌、青紫舌为主要表现,3组间差异有统计学意义(P<0.05)。舌下瘀象及津液方面,高血压合并冠心病组病人舌下瘀象、舌偏润病人占比高于单纯高血压组(P<0.05)。饮食偏嗜方面,高血压合并冠心病组饮食偏咸者占比较高,单纯高血压组次之,健康对照组饮食适中者最高,3组间差异有统计学意义(P<0.05)。3组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平比较,差异均有统计学意义(P<0.001)。结论:高血压合并冠心病病人在舌象特征上与单纯高血压病人、健康人均存在不同程度的差异,结合实验室指标、饮食偏嗜方面等差异可为�ObjectiveObjective:To analyze the tongue characteristics of patients with hypertension and hypertension combined with coronary artery disease,and to provide an objective reference basis for the identification and treatment of patients with hypertension combined with coronary artery disease by combining the laboratory indexes and other characteristics.MethodsMethods:A total of 101 inpatients were selected and divided into the simple hypertension group(65 cases)and hypertension combined with coronary heart disease(36 cases).In the same period,50 healthy subjects in community physical examination center were used as healthy control group.The"TCM Tongue Diagnosis AI Open Platform"was used to collect tongue image,interpret tongue image and extract HSV parameters.ResultsResults:In terms of HSV parameters,the H values of the apical,middle and border parts of the tongue in the healthy control group were higher than those in the simple hypertension group and the hypertension combined with CHD group(P<0.05 or P<0.01),and the S values in all parts of the tongue(including the apex,the middle,the root,and the border parts of the tongue)were lower than those in the simple hypertension group and the hypertension combined with CHD group(P<0.05 or P<0.01).The S values of tongue tip and middle saturation in hypertension combined with CHD were higher than those in simple hypertension group(P<0.05 or P<0.01).The H values of the central and root parts of tongue in healthy control group were higher than those in simple hypertension group and hypertension combined with CHD group(P<0.05 or P<0.01),and the S values in all parts of tongue were lower than those in simple hypertension group and hypertension combined with CHD group(P<0.05 or P<0.01).The H values of the middle and root parts of the tongue in the hypertension combined with CHD group were lower than those in the simple hypertension group(P<0.05 or P<0.01),and the S values in the tip,root and side of tongue were higher than those in the simple hypertension group(P<0.05 or P<0.01
