胆囊结石腹腔镜胆囊切除术患者术前心理弹性现状及其影响因素分析

摘　　要：目的 分析胆囊结石腹腔镜胆囊切除术(LC)患者术前心理弹性的影响因素。方法 87例行LC的胆囊结石患者,均于2021年6月-2024年6月被兰陵县人民医院收治,以心理弹性量表(CD-RISC)评估患者术前心理弹性情况,另收集患者的年龄、性别等资料,探究影响其心理弹性水平的高危因素。计数资料以χ^(2)检验,正态计量资料以t检验。结果 87例行LC的胆囊结石患者,其CD-RISC评分为57.35±4.68分,处在中等水平;单因素分析显示:性别、吸烟史、饮酒史、高血压、糖尿病、冠心病、居住地、婚姻状态、结石数量、病程与行LC的胆囊结石患者心理弹性水平无关,差异无统计学意义(P>0.05);年龄、文化水平、对疾病了解情况、家庭平均月收入、医疗费用支付方式、社会支持度与行LC的胆囊结石患者心理弹性水平有关,差异有统计学意义(P<0.05);多元线性回归分析显示:年龄>60岁(B=1.479,SE=0.601)、高中及以下文化(B=1.630,SE=0.672)、对疾病不了解(B=1.259,SE=0.563)、家庭平均月收入<4 000元(B=1.085,SE=0.398)、医疗费用自费(B=1.987,SE=0.872)、社会支持低水平(B=1.172,SE=0.429)是影响胆囊结石LC患者心理弹性水平的高危因素(P<0.05)。结论 年龄>60岁、高中及以下文化、对疾病不了解、家庭平均月收入<4 000元、医疗费用自费、社会支持低水平是影响胆囊结石LC患者心理弹性水平的独立危险因素。因此,临床需要对以上人群加以关注,并尽早进行个体化的干预对策,以提高患者的心理弹性水平。Objective To analyze the influencing factors of preoperative resilience in patients undergoing laparoscopic cholecystectomy(LC)for gallstones.Methods 87 patients with gallstones undergoing LC were admitted to Lanling County People's Hospital from June 2021 to June 2024.The preoperative resilience of the patients was assessed with the Connor-Davidson resilience scale(CD-RISC),and the age,gender and other data of the patients were also collected to explore the risk factors affecting their resilience.The count data were analyzed by x’test,and the normal measurement data were analyzed by t test.Results The CD-RISC score of 87 LC patients was 57.35±4.68,which was in the medium level.Univariate analysis showed that gender,smoking history,drinking history,hypertension,diabetes,coronary heart disease,place of residence,marital status,stone number and disease course had no significant difference with the resilience of LC patients(P>0.05).Age,education level,understanding of disease,average monthly family income,payment method of medical expenses and social support were correlated with resilience of LC patients,with statistical differences(P<0.05).Multiple linear regression analysis shows:Age>60 years old(B=1.479,SE=0.601),high school education or below(B=1.630,SE=0.672),no knowledge of diseases(B=1.259,SE=0.563),average monthly family income<4000 yuan(B=1.085,SE=0.563,SE=0.398),out-of-pocket medical expenses(B=1.987,SE=0.872)and low level of social support(B=1.172,SE=0.429)were the high risk factors affecting the resilience of LC patients with gallstones(P<0.05).Conclusion Age>60 years old,high school education or below,lack of knowledge about the disease,average monthly family income<4000 yuan,out-of-pocket medical expenses and low level of social support are independent risk factors affecting the level of resilience in patients with LC.Therefore,the clinical need to pay attention to the above population,and as soon as possible to conduct individualized intervention countermeasures,in order to improve the psychologica
