摘 要:目的了解血清维生素D水平在普通尘肺患者与尘肺合并肺部感染患者中的差异,分析血清维生素D水平与感染指标、病原菌之间的相关性。方法选取2023年2月至2024年1月在新疆维吾尔自治区第三人民医院(新疆维吾尔自治区职业病医院)职业病科住院的尘肺合并肺部感染患者120例为研究组,另选取同期门诊体检的50例普通尘肺患者为对照组,比较两组血清维生素D水平,同时分析研究组维生素D水平与白细胞(WBC)、中性粒细胞百分比(NEUT%)、红细胞沉降率(ESR)、C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素(IL)-6及病原菌之间的相关性。结果两组维生素D水平不同的人员分布比较,研究组血清维生素D正常人数占比明显低于对照组,缺乏人数占比明显高于对照组,差异均有统计学意义(χ^(2)=37.006,P<0.001;χ^(2)=20.645,P<0.001)。研究组血清维生素D水平与WBC、NEUT%呈弱负相关,与CRP、IL-6呈中度负相关,与PCT、ESR不相关。研究组不同血清维生素D水平尘肺合并肺部感染患者的细菌和真菌感染情况差异有统计学意义(P<0.05),而耐药菌感染情况差异无统计学意义(P>0.05)。结论尘肺合并感染患者相较于普通尘肺患者血清维生素D水平缺乏率更高,尘肺患者体内的维生素D可能通过调节炎症因子水平参与炎症反应。缺乏维生素D更容易增加呼吸系统细菌、真菌感染率。Objective To understand the difference of serum vitamin D level between common pneumoconiosis patients and pneumoconiosis patients complicated by pulmonary infection,and analyze the correlation between serum vitamin D level and infection indices and pathogenic bacteria.Methods Patients with pneumoconiosis complicated by pulmonary infection who were hos pitalized in the Department of Occupational Disease of the Third People’s Hospital of Xinjiang Uygur Autonomous Region(Xinjiang Uygur Autonomous Region Occupational Disease Hospital)from February 2023 to January 2024 were selected as the study group(n=120),and patients with common pneumoconiosis who underwent physical examination in the same period were selected as the control group(n=50).The serum vitamin D levels of the two groups were compared,and the correlation between the vitamin D levels and white blood cell(WBC),neutrophil percentage(NEUT%),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),procalcitonin(PCT),interleukin(IL)-6 and pathogenic bacteria in the study group was simultaneously analyzed.Results The distributions of people with different levels of vitamin D were compared between the two groups.The proportion of normal serum vitamin D in the study group was obviously lower than that in the control group,and the proportion of deficiency was obviously higher than that in the control group,with statistically significant differences(χ^(2)=37.006,P<0.001,χ^(2)=20.645,P<0.001).The serum vitamin D level in the study group was weakly negatively correlated with WBC and NEUT%,moderately negatively correlated with CRP and IL-6,and not correlated with PCT and ESR.There were statistically significant differences in bacterial and fungal infections among pneumoconiosis patients with different levels of serum vitamin D in the study group(P<0.05),while there was no statistically significant difference in drug-resistant bacterial infections(P>0.05).Conclusion The deficiency rate of serum vitamin D level in pneumoconiosis patients complicated by infection
