苍附导痰汤联合来曲唑治疗排卵障碍性不孕症临床研究

摘　　要：目的:观察苍附导痰汤联合来曲唑治疗排卵障碍性不孕症的临床疗效。方法:选择雅安市第二人民医院2022年7月至2024年7月收治的80例排卵障碍性不孕症患者为研究对象,按照随机数字表法分为对照组和试验组,每组各40例。对照组给予来曲唑治疗,试验组给予苍附导痰汤联合来曲唑治疗。比较两组患者排卵率、妊娠率及治疗前后中医证候积分、激素水平[促黄体生成素(luteinizing hormone,LH)、卵泡刺激素(follicle-stimulating hormone,FSH)、LH/FSH、睾酮(testosterone,T)、雌二醇(estradiol,E2)、孕酮(progesterone,P)]变化情况。结果:治疗后,试验组LH为(6.29±0.24) IU·L^(-1)、FSH(6.87±0.23) IU·L^(-1)、LH/FSH(0.76±0.14),T(0.32±0.12) nmol·L^(-1)、E2(151.22±3.44) ng·L^(-1),P(9.52±1.41)μg·L^(-1),与对照组比较,差异具有统计学意义(P<0.05)。试验组排卵率为62.50%,妊娠率为67.50%,均高于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,治疗后试验组中医证候积分为(15.43±1.42)分,低于对照组的(19.21±2.14)分,差异具有统计学意义(P<0.05)。结论:苍附导痰汤联合来曲唑治疗排卵障碍性不孕症,可改善患者的中医证候及激素水平,提高排卵率及妊娠率。Objective:To observe the clinical efficacy of Rhizoma Atractylodis and Rhizoma Cyperi Phlegm-Expelling Decoction combined with Letrozole on ovulatory disorder infertility.Methods:A total of 80 patients with ovulatory disorder infertility.admitted to Ya′an Second People′s Hospital from July 2022 to July 2024 were selected as the research subjects and randomly divided into the control group and the experimental group,with 40 cases in each group.The control group was treated with Letrozole,while the experimental group was treated with Rhizoma Atractylodis and Rhizoma Cyperi Phlegm-Expelling Decoction combined with Letrozole.The changes in ovulation rate,pregnancy rate,TCM syndrome scores,and hormone levels [such as luteinizing hormone(LH),follicle-stimulating hormone(FSH),LH/FSH,testosterone(T),estradiol(E2),and progesterone(P)] before and after treatment were compared between the two groups.Results:After treatment,the levels of LH,FSH,LH/FSH,T,E2 and P in the experimental group were(6.29±0.24) IU·L^(-1),(6.87±0.23) IU·L^(-1),(0.76±0.14),(0.32±0.12) nmol·L^(-1),(151.22±3.44) ng·L^(-1) and(9.52±1.41) μg·L^(-1) respectively.Compared with the control group,the differences were statistically significant(P<0.05).The ovulation rate and pregnancy rate in the experimental group were 62.50% and 67.50% respectively,both higher than those in the control group,and the differences were statistically significant(P<0.05).The TCM syndrome scores of both groups were lower than those before treatment.The TCM syndrome score of the experimental group after treatment was(15.43±1.42) points,which was lower than that of the control group:(19.21±2.14) points,and the difference was statistically significant(P<0.05).Conclusion:Rhizoma Atractylodis and Rhizoma Cyperi Phlegm-Expelling Decoction combined with Letrozole can improve the TCM syndromes and hormone levels of patients with ovulatory disorder infertility,increase their ovulation rate and pregnancy rate.
