中药热敷联合中药保留灌肠治疗血液透析慢性肾脏病相关性瘙痒症临床研究

摘　　要：目的:观察中药热敷联合中药保留灌肠治疗血液透析慢性肾脏病相关性瘙痒症的临床疗效。方法:将82例在上海中医药大学附属第七人民医院进行血液透析的慢性肾脏病相关性瘙痒症患者随机分为对照组与观察组,每组各41例。对照组采用降酐合剂保留灌肠,观察组在对照组基础上进行中药热敷,观察两组患者治疗后临床疗效及治疗前后视觉模拟量表(visual analog scale,VAS)、5-D瘙痒症量表(5-Ditch scale,5D-IS)评分和瘙痒程度。结果:治疗后,观察组VAS、5D-IS评分降低、瘙痒积分降低,且观察组低于对照组,差异均有统计学意义(P<0.05);对照组有效率为95.13%,观察组有效率为100.00%,两组有效率比较,差异有统计学意义(P<0.05)。结论:中药热敷联合中药保留灌肠治疗血液透析慢性肾脏病相关性瘙痒症可有效缓解患者瘙痒症状,提高生活质量。Objective:To observe the clinical efficacy of Chinese herbal hot compress combined with Chinese herbal retention enema on chronic kidney disease-related pruritus in hemodialysis patients.Methods:A total of 82 patients with chronic kidney disease-related pruritus undergoing hemodialysis at Shanghai Seventh People′s Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were randomly divided into the control group and the observation group,with 41 cases in each group.The control group was treated with Jianggan Mixture retention enema,while the observation group received Chinese herbal hot compress on the basis of the control group′s treatment.The clinical efficacy,changes in visual analog scale(VAS),5-Dimensional Pruritus Scale(5D,S) scores,and pruritus degree of the two groups before and after treatment were compared.Results:After treatment,the VAS,5D,S,and pruritus scores in the observation group were all decreased,and the observation group′s above indicators were lower than those of the control group,with statistically significant differences(P<0.01).The effective rate of the control group was 95.13%,while that of the observation group was 100.00%,with a statistically significant difference(P<0.05).Conclusion:Chinese herbal hot compress combined with Chinese herbal retention enema can effectively relieve pruritus symptoms in hemodialysis patients with chronic kidney disease and improve their quality of life.
