单侧与双侧经皮穿刺椎体成形术对腰椎骨质疏松性椎体骨折患者手术相关指标影响的对比分析

摘　　要：目的:对比单侧经皮穿刺椎体成形术(PVP)与双侧PVP对腰椎骨质疏松性椎体骨折患者的影响。方法:回顾性分析2021年7月—2024年6月菏泽市中医医院收治的90例腰椎骨质疏松性椎体骨折患者资料,根据治疗方式的不同将其分为对照组(n=30)和研究组(n=60)。对照组采用双侧PVP治疗,研究组采用单侧PVP治疗。比较两组手术相关指标、腰椎功能、疼痛程度及生活质量。结果:研究组的术中出血量、骨水泥推注量均少于对照组,手术时间短于对照组,差异均有统计学意义(P<0.05)。术后3个月,两组的Oswestry功能障碍指数、疼痛视觉模拟评分均低于术前,差异均有统计学意义(P<0.05);但两组间比较,差异均无统计学意义(P>0.05)。术后3个月,研究组的生活质量综合评定问卷-74中各个维度评分均高于对照组,差异均有统计学意义(P<0.05)。结论:腰椎骨质疏松性椎体骨折患者采用单侧与双侧PVP治疗均可缓解术后疼痛、改善腰椎功能。与双侧PVP相比,单侧PVP能缩短手术时间,减少术中出血量,提高患者生活质量。Objective:To compare the effects of unilateral percutaneous vertebroplasty(PVP)and bilateral PVP on patients with lumbar osteoporotic vertebral fracture.Methods:Data of 90 lumbar osteoporotic vertebral fracture patients admitted to Heze Traditional Chinese Medicine Hospital from July 2021 to June 2024 were retrospectively analyzed,according to the different treatment methods,they were divided into a control group(n=30)and a study group(n=60).The control group was treated with bilateral PVP,and the study group was treated with unilateral PVP.The surgery-related indexes,lumbar function,pain degree and quality of life were compared between the two groups.Results:The intraoperative blood loss and bone cement injection volume in the study group were less than those in the control group,and the operative time was shorter than that in the control group,the differences were statistically significant(P<0.05).Three months after surgery,the scores of Oswestry Disability Index and Pain Visual Analogue Scale in the two groups were lower than before surgery,the differences were statistically significant(P<0.05);but there were no significant differences between the two groups(P>0.05).Three months after surgery,the scores of all dimension Generic Quality of Life Inventory-74 in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Both unilateral and bilateral PVP can relieve postoperative pain and improve lumbar function in patients with lumbar osteoporotic vertebral fracture.Compared with bilateral PVP,unilateral PVP can shorten operation time and reduce intraoperative blood loss,and improve patients'quality of life.
