MIPPO技术联合锁定钢板对胫骨远端骨折患者的临床治疗效果分析

摘　　要：目的:分析经皮微创钢板内固定(MIPPO)技术联合锁定钢板治疗胫骨远端骨折患者的效果。方法:选择2019年3月—2023年9月延边朝鲜族自治州朝医医院收治的72例胫骨远端骨折患者为研究对象,以随机数字表法将其分为对照组和研究组,各36例。对照组采用锁定钢板内固定术治疗,研究组采用MIPPO技术联合锁定钢板治疗。比较两组骨折愈合情况、围手术期指标、并发症发生情况、踝关节功能。结果:研究组术中出血量较对照组少,住院时间较对照组短,差异均有统计学意义(P<0.05);两组手术时间对比,差异无统计学意义(P>0.05)。研究组骨折愈合优良率为97.22%,高于对照组的77.77%,差异有统计学意义(P<0.05)。术后6个月,两组美国矫形外科足踝协会踝-后足功能评分均高于术前且研究组高于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.78%,低于对照组的25.00%,差异有统计学意义(P<0.05)。结论:胫骨远端骨折患者采用MIPPO技术联合锁定钢板治疗可减少术中出血量,缩短住院时间,改善骨折愈合情况,降低并发症发生率。Objective:To analyze the efficacy of minimally invasive percutaneous plate osteosynthesis(MIPPO)technique combined with locking plate in the treatment of patients with distal tibial fractures.Methods:A total of 72 patients with distal tibial fracture admitted to Chaoyi Hospital of Yanbian Korean Autonomous Prefecture from March 2019 to September 2023 were selected as the study objects,and were divided into a control group and a study group by random number table method,with 36 cases in each group.The control group was treated with locking plate internal fixation,and the study group was treated with MIPPO technique combined with locking plate.Fracture healing,perioperative indexes,the occurrence of complications and ankle joint function were compared between the two groups.Results:The amount of intraoperative blood loss in the study group was less than that in the control group,and the length of hospitalization was shorter,with statistical significance(P<0.05),there was no significant difference in operation time between the two groups(P>0.05).The rate of fracture healing in the study group was 97.22%,which was higher than 77.77%in the control group,the difference was statistically significant(P<0.05).Six months after surgery,the American Orthopaedic Foot and Ankle Society ankle-posterior foot function score in both groups were higher than those before suryey,and the study group was higher than the control group,with statistical significance(P<0.05).The complication rate of the study group was 2.78%,which was lower than 25.00%of the control group,the difference was statistically significant(P<0.05).Conclusion:MIPPO technique combined with locking plate can reduce intraoperative blood loss,shorten hospital stay,improve fracture healing and reduce complication rate in patients with distal tibial fracture.
