摘　　要：目的:探究β2微球蛋白(β2-MG)和尿微量白蛋白(UMA)联合检验在糖尿病早期肾病患者诊断中的应用价值。方法:选择2021年1月—2023年12月天津市和平区五大道街社区卫生服务中心和天津市第五中心医院收治的60例糖尿病早期肾损伤患者作为研究组,以及同期60例健康体检者作为对照组。患者入院后均采取β2-MG、UMA联合检验,将早期肾活检视为金标准,分析β2-MG、UMA单独检测及联合诊断效果,评估不同检验的特异性、灵敏度及准确性。结果:研究组β2-MG、UMA水平均高于对照组,差异均有统计学意义(P<0.05)。以早期肾活检为金标准,β2-MG、UMA联合检验阳性检出率均高于单独检验,差异均有统计学意义(P<0.05)。分析不同检验方式的诊断效能,结果显示,联合检验的诊断效能较高(P<0.05)。结论:β2-MG、UMA联合检验应用于糖尿病早期肾损伤诊断,准确性较高,能够为糖尿病早期肾损伤患者肾损伤状态评估及制订治疗方案提供可靠依据,有一定的应用价值。Objective:To explore the application value ofβ2 microglobulin(β2-MG)and urinary microalbumin(UMA)combined test in the diagnosis of early diabetic nephropathy patients.Methods:A total of 60 diabetic patients with early kidney injury admitted in Wudadao Street Community Health Service Center Tianjin Heping District and Tianjin Fifth Central Hospital from January 2021 to December 2023 were selected as the study group,and 60 healthy subjects who underwent physical examination during the same period were selected as the control group.β2-MG and UMA combined tests were taken after admission,and early kidney biopsy was regarded as the gold standard.The effects ofβ2-MG and UMA alone and combined diagnosis were analyzed,and the specificity,sensitivity and accuracy of different test measures were evaluated.Results:The levels ofβ2-MG and UMA in study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Using early renal biopsy as the gold standard,the positive detection rate ofβ2-MG and UMA combined test were higher than those of single test,and the differences were statistically significant(P<0.05).The diagnostic efficacy of different test methods was analyzed,the results showed that the diagnostic efficacy of the combined test was higher(P<0.05).Conclusion:The combined test ofβ2-MG and UMA can be used in the diagnosis of early diabetic kidney injury with high diagnostic accuracy,and can provide a reliable basis for the assessment of kidney injury status and the formulation of treatment plan for patients with early diabetic kidney injury,and has certain application value.
