大隐静脉高位结扎联合腔内激光与点式抽剥术治疗对中重度下肢静脉曲张患者围手术期指标及应激反应的影响

摘　　要：目的:探究中重度下肢静脉曲张患者行大隐静脉高位结扎联合腔内激光与点式抽剥术对围手术期指标及应激反应的影响。方法:回顾性分析2021年3月—2024年4月沂水县精神卫生中心收治的84例中重度下肢静脉曲张患者资料,根据手术方式的不同将其分为对照组(n=41)和研究组(n=43),对照组采用大隐静脉高位结扎和点式抽剥术治疗,研究组采用大隐静脉高位结扎联合腔内激光与点式抽剥术治疗。对比两组围手术期指标、应激反应、病情严重程度、并发症发生情况、生活质量。结果:研究组手术时间、术后首次下床活动时间、住院时间均短于对照组,术中出血量少于对照组,术后3d疼痛视觉模拟评分低于对照组,差异均有统计学意义(P<0.05)。术后24h,研究组超氧化物歧化酶高于对照组,丙二醛低于对照组,差异均有统计学意义(P<0.05)。术后3个月,研究组静脉临床严重程度评分低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率为4.65%,低于对照组的21.95%,差异有统计学意义(P<0.05)。术后3个月,研究组生活质量综合评定问卷-74各个维度评分均高于对照组,差异均有统计学意义(P<0.05)。结论:中重度下肢静脉曲张患者行大隐静脉高位结扎联合腔内激光与点式抽剥术联合治疗可优化围手术期指标,减轻应激反应及静脉曲张程度,减少并发症,提高生活质量。Objective:To investigate the effects of combined treatment of high ligation of the great saphenous vein combined with intracavitary laser and point extraction technique on perioperative indicators and stress response in patients with moderate to severe varicose veins in the lower limbs.Methods:A retrospective analysis was conducted on the data of 84 patients with moderate to severe varicose veins in the lower limbs admitted to Yishui County Mental Health Center from March 2021 to April 2024,according to different surgical methods they were divided into a control group(n=41)and a study group(n=43).The control group were adopted high ligation of the great saphenous vein and point extraction technique treatment,and the study group adopted high ligation of the great saphenous vein combined with intracavitary laser and point extraction technique treatment.The perioperative indicators,stress response,severity of illness,incidence of complications,and quality of life were compared between the two groups.Results:The surgical time,first time getting out of bed after surgery,hospitalization time in the study group were shorter than those in the control group,the amount of intraoperative blood loss was less than that in the control group,and postoperative 3 d the score of Pain Visual Analogue Scale was lower than that in the control group,with statistical difference(P<0.05).Twenty-four hours after surgery,the level of superoxide dismutase of the study group was lower than that in the control group,the levels of malondialdehyde of the study group was higher than that in the control group,with statistical difference(P<0.05).Three months after surgery,the Venous Clinical Severity Score in the study group was lower than that in the control group,with statistical difference(P<0.05).The incidence of complications in the study group was 4.65%,which was lower than 21.95%in the control group,with statistical difference(P<0.05).Three months after surgery,the all dimensions score of Generic Quality of Life Inventory-74 in the study gr
