舌针联合吞咽康复训练治疗老年中风后吞咽障碍患者的效果

摘　　要：目的:探讨舌针联合吞咽康复训练治疗老年中风后吞咽障碍患者的效果。方法:选取2022年2月—2024年3月临沂市中心医院收治的80例老年中风后吞咽障碍患者为研究对象,采用随机数字表法将其分为对照组和研究组,各40例。对照组采用吞咽康复训练治疗,研究组采用舌针联合吞咽康复训练治疗。两组均治疗4周。比较两组中医症候积分、吞咽功能、不良事件发生情况、生活质量。结果:治疗后,研究组舌强语謇/失语、饮水呛咳、口舌歪斜、肢体麻木的中医症候积分均低于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组洼田饮水试验与标准吞咽功能评估量表评分均低于对照组,吞咽造影检查评分高于对照组,差异均有统计学意义(P<0.05)。研究组不良事件发生率为7.50%与对照组的12.50%比较,差异无统计学意义(P>0.05)。治疗后,两组吞咽生活质量量表评分均高于治疗前,且研究组高于对照组,差异均有统计学意义(P<0.05)。结论:舌针联合吞咽康复训练可改善对老年中风后吞咽障碍患者的吞咽功能,减轻其症候,减少不良事件发生情况,安全性高,有助于提高患者生活质量,具有一定应用价值。Objective:To investigate the effect of tongue acupuncture combined with swallowing rehabilitation training in the treatment of elderly patients with dysphagia after stroke.Methods:A total of 80 elderly patients with dysphagia after stroke treated in Linyi Central Hospital from February 2022 to March 2024 were selected as the study objects,and were divided into control group and study group by random number table method,with 40 cases in each group.The control group was treated with swallowing rehabilitation training,and the study group was treated with tongue acupuncture combined with swallowing rehabilitation training.Both groups were treated for 4 weeks.The scores of traditional Chinese medicine symptoms,swallowing function,occurrence of adverse events and quality of life were compared between the two groups.Results:After treatment,the scores of traditional Chinese medicine syndrome of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,the scores of Water Swallowing Test and Standard Swallowing Assessment Scale in the study group were lower than those in the control group,and the scores of Videofluoroscopic Swallowing Study were higher than those in the control group,with statistical significance(P<0.05).The incidence of adverse events in the study group was 7.50%compared with 12.50%in the control group,with no significant difference(P>0.05).After treatment,the scores of Swallowing Quality of Life Scale in both groups were higher than those before treatment,and those of the study group was higher than those of the control group,with statistical significance(P<0.05).Conclusion:Tongue acupuncture combined with swallowing rehabilitation training can improve the swallowing function,relieve the symptoms,reduce the occurrence of adverse events,high safety,help to improve the quality of life of patients,and have certain application value.
