整合性社区健康服务:以老年人跌倒的风险管理为例

摘　　要：在我国,跌倒是造成65岁及以上人群死亡和伤害的主要原因,也给社会和家庭带来沉重的照料负担和经济损失。生理因素、心理因素、家庭因素和环境因素均对老年人跌倒有一定影响。年龄在65-69岁、担心跌倒、卫生间未安装防滑扶手、与家庭成员关系不和睦是老年人发生跌倒的风险因素。老年人跌倒风险因素的多样性决定了构建整合性社区健康服务的必要性和重要性。基层医疗人员在健康管理中应定期开展社区老年人预防跌倒的健康知识讲座,普及老年人跌倒的风险因素,为患有骨质疏松、高血压等高危人群普及膳食改善、合理用药方面的知识;社区工作者、心理咨询师或志愿者需要针对老年人跌倒恐惧提供相应的心理支持;社区工作者应关注老年人的家庭状况,积极促进家庭和睦关系的建立;政府可通过购买服务或者政府补贴等方式开展居家适老化改造,为老年人提供更加安全的生活环境。In China,falls are a leading cause of death and injury among people aged 65 and above,imposing significant caregiving burdens and economic losses on society and families.Physiological,psychological,familial,and environmental factors all contribute to the risk of falls among the elderly.Risk factors for falls include being aged 65-69,fear of falling,lack of anti-slip handrails in bathrooms,and poor family relationships.The diversity of risk factors for falls among the elderly highlights the necessity and importance of establishing integrated community health services.In health management,primary healthcare providers should regularly conduct health education lectures on fall prevention for elderly community members,raising awareness of fall risk factors and providing knowledge on dietary improvements and rational medication use for high-risk groups such as those with osteoporosis or hypertension.Community workers,psychologists,or volunteers should offer psychological support to address the fear of falling among the elderly.Community workers should also pay attention to the family situations of the elderly and actively promote harmonious family relationships.The government can facilitate home modifications for aging in place through service procurement or subsidies,providing a safer living environment for the elderly.
