锁骨钩形钢板固定术治疗肩锁关节脱位患者的效果观察

摘　　要：目的:探讨运用锁骨钩形钢板固定术治疗肩锁关节脱位患者的效果,并分析其对肩关节功能的影响。方法:选择2022年5月-2023年5月枣庄市峄城区人民医院收治的68例肩锁关节脱位患者为研究对象,采用随机数字表法将其分成对照组和研究组,各34例。对照组采用克氏针张力带联合钢丝与螺钉的固定方法,研究组采用锁骨钩形钢板固定术治疗。比较两组手术相关指标、肩关节功能、肩关节活动度、并发症发生情况。结果:研究组愈合时间、住院时间、手术时间均短于对照组,术中出血量低于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组Constant-Murley肩关节功能评分量表评分高于对照组,肩关节前屈、外展活动度均大于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率2.94%低于对照组的20.59%,差异有统计学意义(P<0.05)。术后1、3个月,研究组肩关节主观评分系统中疼痛视觉模拟评分均低于对照组,累计日常活动评分均高于对照组,差异均有统计学意义(P<0.05)。结论:锁骨钩钢板在肩锁关节脱位的治疗中能有效改善患者手术时相关指标和肩关节功能及其活动度,利于术后肩关节功能的恢复,减少并发症的发生,值得临床应用。Objective:To investigate the effect of clavicular hook platefixation in the treatment of patients with acromioclavicular joint dislocation and analyze the effect on shoulder joint function.Methods:A total of 68 patients with acromioclavicular joint dislocation admitted to Yicheng District People’s Hospital of Zaozhuang City from May 2022 to May 2023 were selected as the study objects.They were divided into control group and study group by random number table method,with 34 cases in each group.The control group wasfixed with Kirschner needle tension band combined with steel wire and screw,and the study group wasfixed with clavicular hook plate.The operative indexes,shoulder joint function,shoulder joint range of motion and the occurrence of complications were compared between the two groups.Results:The healing time,hospitalization time and operation time of the study group were shorter than those of the control group,and the amount of blood loss during operation was lower than that of the control group,with statistical significance(P<0.05).After treatment,the Constant-Murley shoulder function scale score of the study group was higher than that of the control group,and the shoulderflexion and abduction motion were higher than that of the control group,with statistical significance(P<0.05).The complication rate of the study group was 2.94%,which was lower than 20.59%of the control group,the difference was statistically significant(P<0.05).One and three months after surgery,the visual analogue scores of pain of Subjective Shoulder Rating Systen in the study group were lower than those in the control group,and the cumulative daily activity scores were higher than those in the control group,with statistical significance(P<0.05).Conclusion:Clavicular hook plate in the treatment of acromioclavicular joint dislocation can effectively improve the relevant indexes and shoulder joint function and mobility of patients during surgery,which is conducive to the recovery of shoulder joint function after surgery and r
