布比卡因蛛网膜下腔阻滞与全身麻醉在髋关节置换术患者中的应用效果比较

摘　　要：目的:比较布比卡因蛛网膜下腔阻滞与全身麻醉在髋关节置换术患者中的应用效果。方法:选取2023年1—12月该院收治的108例髋关节置换术患者进行前瞻性研究,按照随机数字表法将其分为研究组与对照组各54例。对照组采用全身麻醉,研究组采用布比卡因蛛网膜下腔阻滞,比较两组舒张压、收缩压、手术时间、术中出血量、术后住院时间、术后首次下床时间、Harris髋关节功能评分量表(HHS)评分和心脑血管并发症发生率。结果:研究组术后首次下床时间短于对照组,差异有统计学意义(P<0.05);术后2周,两组HHS评分高于术前,且研究组高于对照组,差异均有统计学意义(P<0.05);麻醉前、手术结束时,两组舒张压和收缩压组间比较,差异均无统计学意义(P>0.05);两组手术时间、术中出血量、术后住院时间、心脑血管并发症发生率比较,差异均无统计学意义(P>0.05)。结论:布比卡因蛛网膜下腔阻滞应用于髋关节置换术患者可缩短术后首次下床时间,以及提高HHS评分,效果优于全身麻醉。Objective:To compare application effects of Bupivacaine subarachnoid block and general anesthesia in patients undergoing hip replacement.Methods:A prospective study was conducted on 108 patients with hip replacement admitted to this hospital from January to December 2023.According to the random number table method,they were divided into study group and control group,54 cases in each group.The control group was treated with general anesthesia,while the study group was treated with Bupivacaine subarachnoid block.The levels of diastolic blood pressure,systolic blood pressure,operation time,intraoperative blood loss,postoperative hospitalization time and first out-of-bed time after the surgery,the Harris hip score(HHS)score,and the incidence of cardiovascular and cerebrovascular complications were compared between the two groups.Results:The first out-of-bed time after the surgery in the study group was shorter than that in the control group,and the difference was statistically significant(P<0.05).Two weeks after the surgery,the HHS scores of the two groups were higher than those before the surgery,that in the study group was higher than that in the control group,and the differences were statistically significant(P<0.05).Before anesthesia and at the end of the surgery,there were no significant differences in the levels of diastolic blood pressure and systolic blood pressure between the two groups(P>0.05).There were no significant differences in the operation time,the intraoperative blood loss,the postoperative hospitalization time and the incidence of cardiovascular and cerebrovascular complications between the two groups(P>0.05).Conclusions:The Bupivacaine subarachnoid block in the patients undergoing hip replacement can shorten the first out-of-bed time after the surgery and improve the HHS scores.Moreover,it is superior to the general anesthesia.
