德谷门冬双胰岛素联合二甲双胍治疗2型糖尿病的临床效果及对胰岛素抵抗的影响

摘　　要：目的探讨德谷门冬双胰岛素联合二甲双胍治疗2型糖尿病(T2DM)的临床效果及对胰岛素抵抗水平的影响。方法选择2022年3月至2024年3月我院收治的90例T2DM患者按随机数字表法分为对照组和观察组,每组45例。对照组采用二甲双胍治疗,观察组采用德谷门冬双胰岛素联合二甲双胍治疗,比较两组患者的临床效果、胰岛功能[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、空腹胰岛素(FINS)]、糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbAlc)]及不良反应。结果观察组的临床效果优于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者的FINS、HOMA-β、HOMA-IR及FPG、2hPG、HbAlc指标均较治疗前明显改善,且观察组的HOMA-IR、FINS、FPG、HbAlc、2hPG水平均低于对照组,HOMA-β高于对照组,差异均有统计学意义(均P<0.05)。两组患者的不良反应比较,差异无统计学意义(P>0.05)。结论在二甲双胍治疗基础上联合德谷门冬双胰岛素治疗T2DM可显著提高临床治疗效果,改善胰岛功能及糖代谢指标,且不增加不良反应,值得临床推广应用。Objective To investigate the clinical effect of Degludec insulin combined with metformin in the treatment of type 2 diabetes mellitus(T2DM)and its influence on insulin resistance.Methods Ninety T2DM patients admitted to our hospital from March 2022 to March 2024 were randomly divided into a control group and an observation group using a random number table,with 45 patients in each group.The control group was treated with metformin,while the observation group was treated with a combination of Degludec insulin and metformin.Compare the clinical outcomes of two groups of pancreatic function[HOMA-IR,HOMA-β,FINS],glucose metabolism indicators[FPG,PBG,HbAlc],and drug safety.Results The clinical effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the FINS,HOMA-β,HOMA-IR,FPG,PBG,and HbAlc indicators of both groups of patients were significantly improved compared to before treatment,and the HOMA-IR,FINS,FPG,HbAlc,and 2hPG levels of the observation group were lower than those of the control group on average,while HOMA-βwas higher than that of the control group,and the differences were statistically significant(all P<0.05).There was no statistically significant difference in drug safety between the two groups of patients(P>0.05).Conclusion Combining Degludec Insulin with metformin treatment for T2DM can significantly improve clinical efficacy,improve pancreatic function and glucose metabolism indicators,without increasing adverse reactions,and is worthy of promotion and application.
