分娩恐惧、社会支持对妊娠晚期孕妇产后创伤后应激障碍的影响

摘　　要：目的:调查妊娠晚期孕妇产后创伤后应激障碍(PP-PTSD)发生水平及分娩恐惧、社会支持水平,探索分娩恐惧、社会支持对PP-PTSD的作用路径。方法:采用方便抽样调查2022年7月-2023年6月就诊于某院的300例妊娠晚期孕妇,使用Pearson相关分析法检验分娩恐惧、社会支持与PP-PTSD之间的相关性,利用逐步回归法检验分娩恐惧在社会支持与PP-PTSD间的中介效应,检验水准α取0.05。结果:300例孕妇,分娩恐惧得分为34.65±9.29分,分娩恐惧发生率为71.33%(214/300);社会支持得分为28.93±10.93分,社会支持水平高的占17.33%(52/300);PP-PTSD得分为31.75±6.61分,PP-PTSD发生率为24.33%(73/300)。且分娩恐惧是社会支持对PP-PTSD作用关系中的中介变量,且为完全中介,中介作用的间接效应占比为97.58%。结论:妊娠晚期孕产妇PP-PTSD发生率较高,且社会支持能通过分娩恐惧作用于PP-PTSD。医疗卫生机构应重视妊娠晚期孕产妇PP-PTSD问题,通过针对性措施降低PP-PTSD的发生概率。Objective:To investigate the occurrence level of postpartum post-traumatic stress disorder(PP-PTSD)and the levels of fear of delivery and social support in late pregnant women,and to explore the effect path of fear of delivery and social support on PP-PTSD.Methods:A convenient sampling survey was conducted among 300 late pregnant women in our hospital from July 2022 to June 2023.Pearson correlation analysis was used to test the correlation between fear of childbirth,social support and PP-PTSD.Stepwise regression was used to test the mediating effect of fear of childbirth between social support and PP-PTSD.The test levelαwas 0.05.Results:Among 300 pregnant women,the score of fear of delivery was(34.65±9.29),and the incidence of fear of delivery was 71.33%(214/300);The score of social support was(28.93±10.93),and the high level of social support accounted for 17.33%(52/300);The score of PP-PTSD was(31.75±6.61),and the incidence of PP-PTSD was 24.33%(73/300).The fear of childbirth was the mediating variable in the relationship between social support and PP-PTSD,and it was a complete mediator.The indirect effect of the mediating effect accounts for 97.58%.Conclusion:The incidence of PP-PTSD in late pregnancy is high,and social support can affect PP-PTSD through childbirth fear.Medical and health institutions should pay attention to the problem of PP-PTSD in late pregnancy and reduce the probability of PP-PTSD through targeted measures.
