摘 要:目的 探讨高频超声联合剪切波弹性成像技术对单侧跟腱断裂康复后受伤跟腱及对侧跟腱形态变化的诊断价值。方法 选取47例单侧跟腱断裂冰雪运动爱好者受伤侧跟腱为实验组,47例对侧无症状跟腱为对照组,31名健康志愿者起跳腿侧正常跟腱为健康对照组。采用高频超声及剪切波弹性成像技术分别测量研究对象跟腱近段、中段、远段的厚度、横截面积、杨氏模量值及剪切波速度。结果 冰雪运动爱好者单侧跟腱断裂后,受伤跟腱及对侧无症状跟腱的近段厚度及全横截面积显著高于健康对照组(P<0.05),中、远段杨氏模量值和近段剪切波速度显著低于健康对照组(P<0.05)。结论 冰雪运动爱好者单侧跟腱断裂康复后,受伤跟腱及对侧无症状跟腱在形态上表现为增厚和横截面积增大,但硬度和弹性下降,更易受损。高频超声联合剪切波弹性成像技术能够有效评估这些变化,为康复训练和运动强度管理提供科学依据。Objective To investigate the diagnostic value of high frequency ultrasound combined with shear wave elastography in the morphological changes of injured Achilles tendon and contralateral Achilles tendon after rehabilitation of unilateral Achilles tendon rupture.Methods A total of 47 cases of unilateral Achilles tendon rupture in ice and snow sports enthusiasts injured Achilles tendon as the experimental group,47 cases of the contralateral asymptomatic Achilles tendon as the control group,31 healthy volunteers take-off leg side normal Achilles tendon as the healthy control group.The thickness,cross-sectional area,Young’s modulus and shear wave velocity of the proximal,middle and distal Achilles tendon were measured by high frequency ultrasound and shear wave elastography.Results After unilateral Achilles tendon rupture in ice and snow sports enthusiasts,the proximal thickness and total cross-sectional area of the injured Achilles tendon and the contralateral asymptomatic Achilles tendon were significantly higher than those in the healthy control group(P<0.05).The Young’s modulus and proximal shear wave velocity in the middle and distal segments were significantly lower than those in the healthy control group(P<0.05).Conclusion After the rehabilitation of unilateral Achilles tendon rupture in ice and snow sports enthusiasts,the injured Achilles tendon and the contralateral asymptomatic Achilles tendon showed thickening and increased cross-sectional area in morphology,but the hardness and elasticity decreased and were more likely to be damaged.High-frequency ultrasound combined with shear wave elastography can effectively evaluate these changes and provide a scientific basis for rehabilitation training and exercise intensity management.
