摘  要:目的探讨三维容积超声联合血小板反应蛋白-1(TSP-1)、血小板活化因子(PAF)对新生儿颅内动脉出血的诊断价值。方法回顾性分析158例疑似颅内动脉出血的新生儿的临床资料,确诊为颅内动脉出血的90例新生儿为研究组,颅内动脉未出血的68例新生儿为对照组。分析2组新生儿三维容积超声参数及TSP-1、PAF表达水平,探讨三维容积超声参数与TSP-1、PAF的相关性。结果研究组患儿三维容积超声收缩期血流速度(Vs)、舒张期血流速度(Vd)、阻力指数(RI)高于对照组,TSP-1、PAF表达水平低于对照组(P<0.05)。与研究组Ⅰ~Ⅱ期患儿相比,Ⅲ~Ⅳ期患儿Vs、Vd、RI参数值升高(P<0.05),TSP-1、PAF表达水平降低(P<0.05)。Vs、Vd、RI参数值与不同病情分期呈正相关(P<0.05),TSP-1、PAF表达水平与不同病情分期呈负相关(P<0.05);Vs、Vd、RI参数值与TSP-1、PAF表达水平呈负相关(P<0.05)。联合检测对新生儿颅内动脉出血诊断价值高于单项诊断(P<0.05)。结论三维容积超声联合TSP-1、PAF对新生儿颅内动脉出血具有较高的诊断价值,三维容积超声参数值与TSP-1、PAF表达水平呈负相关。Objective To investigate the diagnostic value of three-dimensional volume ultrasound combined with thrombospondin-1(TSP-1)and platelet activating factor(PAF)in neonatal intracranial arterial hemorrhage.Methods The clinical data of 158 neonates with suspected intracranial arterial hemorrhage were retrospectively analyzed.90 neonates diagnosed as intracranial arterial hemorrhage were selected as the study group,and 68 neonates without intracranial arterial hemorrhage were selected as the control group.The three-dimensional volume ultrasound parameters and the expression levels of TSP-1 and PAF in the two groups were examined,and the correlation between the three-dimensional volume ultrasound parameters and TSP-1 and PAF was analyzed.Results The systolic blood flow velocity(Vs),diastolic blood flow velocity(Vd)and resistance index(RI)of three-dimensional volume ultrasound in the study group were higher than those in the control group,and the expression levels of TSP-1 and PAF were lower than those in the control group(P<0.05).Compared with stage I-II children in the study group,the Vs,Vd and RI parameters of stage III-IV children increased(P<0.05),and the expression levels of TSP-1 and PAF decreased(P<0.05).The parameters of Vs,Vd and RI were positively correlated with different disease stages(P<0.05),and the expression levels of TSP-1 and PAF were negatively correlated with different disease stages(P<0.05).The parameters of Vs,Vd and RI were negatively correlated with the expression levels of TSP-1 and PAF(P<0.05).The diagnostic value of combined detection for neonatal intracranial arterial hemorrhage was higher than that of single diagnosis(P<0.05).Conclusion Three-dimensional volume ultrasound combined with TSP-1 and PAF has a high diagnostic value for neonatal intracranial arterial hemorrhage.The parameters of three-dimensional volume ultrasound are negatively correlated with the expression levels of TSP-1 and PAF.
