摘 要:目的 应用脑-颈血管一体化超声分析不同分级、不同病程及不同危险分层的原发性高血压患者血流动力学参数差异。方法 选取100例原发性高血压患者为高血压组,同期健康体检者100例为对照组。应用脑-颈血管一体化超声观测颈总动脉(CCA)、大脑中动脉(MCA)血流动力学参数,比较组间及不同分级、不同病程及不同危险分层的原发性高血压患者血流动力学参数差异。结果 高血压组患者CCA及MCA收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)均低于对照组(P<0.01),而血管阻力指数(RI)及血管搏动指数(PI)均高于对照组(P<0.001)。原发性高血压患者CCA及MCA的PSV及EDV随患者高血压分级、病程及危险层级的增加而增加(P<0.05),RI及PI则随患者高血压分级、病程及危险层级的增加而降低(P<0.05)。结论 脑-颈血管一体化超声可有效评估原发性高血压患者的血流动力学变化情况,有效评估患者病情,为临床诊治提供积极参考。Objective The differences of hemodynamic parameters in patients with essential hypertension with different grades,different course of disease and different risk stratification were analyzed by cerebral-cervical vascular integrated ultrasound.Methods A total of 100 patients with essential hypertension were selected as the hypertension group,and 100 healthy subjects were selected as the control group.The hemodynamic parameters of common carotid artery(CCA)and middle cerebral artery(MCA)were observed by integrated cerebral-cervical vascular ultrasound.The differences of hemodynamic parameters between groups and patients with essential hypertension of different grades,different course of disease and different risk stratification were compared.Results The peak systolic velocity(PSV)and end diastolic velocity(EDV)of CCA and MCA in the hypertension group were lower than those in the control group(P<0.01),while the vascular resistance index(RI)and vascular pulsation index(PI)were higher than those in the control group(P<0.001).The PSV and EDV of CCA and MCA in patients with essential hypertension increased with the increase of hypertension grade,course of disease and risk level(P<0.05),while RI and PI decreased with the increase of hypertension grade,course of disease and risk level(P<0.05).Conclusion Cerebral-cervical vascular integrated ultrasound can be used to effectively evaluate the hemodynamic changes of patients with essential hypertension and the patient's condition,providing a positive reference for clinical diagnosis and treatment.
