基于辨证施治治疗慢性阻塞性肺疾病急性加重期临床疗效分析

摘　　要：目的探讨基于辨证施治原则治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)的临床效果。方法178例AECOPD患者按随机数字表法分为对照组87例与观察组91例,对照组采用常规治疗,观察组在对照组基础上辅以基于辨证施治的中医治疗。治疗3个疗程后,观察患者第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及血气分析、血液流变学指标改善情况。结果治疗3个疗程后,观察组临床总有效率优于对照组(P<0.05),FVC及FEV1改善情况及pH值、高切全血黏度、低切全血黏度、血浆黏度、红细胞比容改善均优于对照组(P<0.05);2组患者均无明显不良反应。结论基于辨证施治原则中药辅助治疗AECOPD,临床疗效显著,可明显改善血液流变学及肺功能指标,且无明显不良反应,值得临床参考应用。Objective To explore the clinical effect of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)based on the principle of syndrome differentiation.Methods A total of 178 patients with AECOPD were divided into the control group(87 cases)and the observation group(91 cases)according to the random number table method.The control group was treated with routine treatment,and the observation group was treated with traditional Chinese medicine based on syndrome differentiation on the basis of the control group.After 3 courses of treatment,the improvement of forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),blood gas analysis and hemorheological indexes were observed.Results After 3 courses of treatment,the total effective rate of the observation group was better than that of the control group(P<0.05).The improvement of FVC and FEV1,pH value,high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity and hematocrit in the observation group were better than those in the control group(P<0.05).There were no obvious adverse reactions in the two groups.Conclusion Based on the principle of syndrome differentiation treatment,traditional Chinese medicine has a significant clinical effect in the treatment of AECOPD,which can significantly improve the hemorheology and lung function indexes without obvious adverse reactions.It is worthy of clinical reference application.
