肺腺癌经气腔播散的CT影像学特征及影像学参数与Ki-67的相关性

摘　　要：目的:研究肺腺癌经气腔播散(STAS)的CT影像学特征,并探讨部分影像学参数与Ki-67的相关性。方法:选取我院2016-2022年经手术病理证实为肺腺癌的65例患者,分为STAS阳性组30例,STAS阴性组35例,比较两组CT影像学特征的差异性,并进一步分析弦弧距/弦长比值、动脉期净增值与Ki-67指数的相关性。结果:STAS阳性组肺纤维化、血管集束征、胸膜凹陷征的发生率高于STAS阴性组(P<0.05),肺内磨玻璃影、支气管充气征的发生率低于STAS阴性组(P<0.05)。肿瘤弦弧距/弦长比值、动脉期净增值与Ki-67指数呈正相关(r=0.78、0.74,P<0.05)。结论:与STAS阴性患者相比,STAS阳性肺腺癌患者CT影像中肺纤维化、血管集束征、胸膜凹陷征的发生率更高,且弦弧距/弦长比值、动脉期净增值与Ki-67指数密切相关。Objective:To study the CT imaging features of spread through air space(STAS)in lung adenocarcinoma,and to explore the correlation between some imaging parameters and Ki-67.Methods:Sixty-five patients with lung adenocarcinoma confirmed by surgical pathology in our hospital from 2016 to 2022 were selected.They were divided into 30 cases in the STAS positive group and 35 cases in the STAS negative group.The differences in CT imaging features between the two groups were compared,and the correlation between the chord-arc distance/chord length ratio,the net increase value in the arterial phase and the Ki-67 index was further analyzed.Results:The incidences of pulmonary fibrosis,vascular convergence sign and pleural indentation sign in the STAS positive group were higher than those in the STAS negative group(P<0.05),and the incidences of ground-glass opacity in the lung and air bronchogram sign in the STAS positive group were lower than those in the STAS negative group(P<0.05).The chord-arc distance/chord length ratio of the tumor and the net increase value in the arterial phase were positively correlated with the Ki-67 index(r=0.78,0.74,P<0.05).Conclusion:Compared with STAS negative patients,the incidences of pulmonary fibrosis,vascular convergence sign and pleural indentation sign in the CT images of STAS positive lung adenocarcinoma patients are higher,and the chord-arc distance/chord length ratio and the net increase value in the arterial phase are closely related to the Ki-67 index.
