摘　　要：目的:应用基于社会生态系统理论的质性研究探讨痛风患者自我管理过程中的真实体验。方法:纳入2023年12月至2024年3月贵阳某三级甲等中医院风湿免疫科的16例痛风患者作为访谈对象,基于社会生态系统理论设置半结构式访谈,运用Colaizzi现象学7步分析法分析访谈资料。结果:患者在自我管理经历中的真实体验,可归纳为3个系统:微观系统--生理及心理因素影响患者自我管理;中观系统--生活模式改变影响患者自我管理;宏观系统--支持系统不足影响患者自我管理。结论:痛风患者的社会生态系统不容乐观,普遍存在较重的经济负担。医护人员需注重患者微观系统调节,协调中观系统,优化宏观系统,促进患者提升自我管理的能力。Objective:To explore the real experience of gout patients in the self-management process through a qualitative study based on the social-ecological system theory.Methods:A total of 16 gout patients from the Rheumatology and Immunology Department of a Class II Grade A traditional Chinese medicine hospital in Guiyang from December 2023 to March 2024 were included as interviewees.Semi-structured interviews were set up based on the social-ecological system theory,and the interview data were analyzed using the Colaizzi's seven-step phenomenological analysis method.Results:The real experiences of patients in the self-management process could be summarized into three systems.The microsystem:physiological and psychological factors affecting patients'self-management;the mesosystem:changes in lifestyle affecting patients'self-management;the macrosystem:insufficient support systems affecting patients'self-management.Conclusion:The social-ecological system of gout patients is not optimistic,and they generally have a heavy economic burden.Medical staff need to pay attention to the regulation of the patients'microsystem,coordinate the mesosystem,optimize the macrosystem,and promote patients'ability to improve self-management.
