益艾康胶囊对HIV/AIDS肺脾气虚证患者树突状细胞焦亡水平的影响

摘　　要：目的探讨HIV/AIDS肺脾气虚证患者外周血中树突状细胞(dendritic cells,DC)焦亡水平的变化及益艾康胶囊治疗对HIV/AIDS肺脾气虚证患者DC焦亡水平的影响。方法筛选2023年3月—2023年12月河南某艾滋病高发地区HIV/AIDS肺脾气虚证患者20例作为研究组,选取同地区健康者20例作为对照组,HIV/AIDS肺脾气虚证患者给予益艾康胶囊治疗6个月。首先比较治疗前两组T淋巴细胞亚群、DC频数及焦亡水平、炎症因子水平、干扰素诱导蛋白16(interferon inducible protein 16,IFI16)/半胱天冬氨酸蛋白酶1(cysteinyl aspartate specific proteinase 1,Caspase-1)/消皮素D(gasdermin D,GSDMD)通路相关分子水平,其次对比益艾康胶囊治疗后以上指标的改变。结果与健康对照组相比,HIV/AIDS肺脾气虚证患者的CD4^(+)T淋巴细胞计数和CD4^(+)/CD8^(+)T淋巴细胞比值降低(P<0.05),CD8^(+)/CD3^(+)T淋巴细胞比值升高(P<0.05);DC频数降低,DC焦亡水平升高(P<0.05);白介素-1β(interleukin 1β,IL-1β)和白介素-18(interleukin 18,IL-18)升高(P<0.05);IFI16、Caspase-1、GSDMD、IL-1β、IL-18 mRNA水平升高(P<0.05)。益艾康胶囊治疗可升高HIV/AIDS肺脾气虚证患者CD4^(+)T淋巴细胞计数和CD4^(+)/CD8^(+)T淋巴细胞比值,降低CD8^(+)/CD3^(+)T淋巴细胞比值(P<0.05);升高DC频数,降低DC焦亡水平(P<0.05);降低IL-1β、IL-18细胞因子含量(P<0.05);降低IFI16、Caspase-1、GSD⁃MD、IL-1β、IL-18 mRNA基因表达(P<0.05)。结论HIV/AIDS肺脾气虚证患者的免疫功能降低,DC频数降低,焦亡相关分子基因及炎症因子水平升高;益艾康胶囊治疗能够显著提升HIV/AIDS肺脾气虚证患者免疫功能,降低DC焦亡水平,提高DC频数的机制可能与调控IFI16/Caspase-1/GSDMD抑制DC焦亡有关。Objective To investigate the changes of peripheral blood dendritic cells(DC)in HIV/AIDS patients with syndrome of qi deficiency of lung and spleen and the effect of Yiaikang capsule on the level of DC in HIV/AIDS patients with syndrome of qi deficiency of lung and spleen.Methods From March 2023 to December 2023,20 HIV/AIDS pa⁃tients with syndrome of qi deficiency of lung and spleen in a high incidence area of Henan Province were selected as the study group,and 20 healthy people in the same area were selected as the control group.HIV/AIDS patients with syn⁃drome of qi deficiency of lung and spleen were treated with Yiaikang capsule for 6 months.T Lymphocyte subsets,DC fre⁃quency,apoptosis,inflammatory factors and interferon induc⁃ible protein 16(IFI16)were compared between the two groups before treatment.IFI16/Caspase-1/Gasdermin D(GSDMD)pathway related molecular levels,secondly,the changes of the above indexes after the treatment of Yiaikang capsule were compared.Results Compared with the healthy control group,the CD4^(+)T lymphocyte count and CD4^(+)/CD8^(+)T lymphocyte ratio in HIV/AIDS patients with syndrome of qi de⁃ficiency of lung and spleen were decreased(P<0.05),while the CD8^(+)/CD3^(+)T lymphocyte ratio was increased(P<0.05),the frequency of DC was decreased,and the pyroptosis level of DC was increased(P>0.05);the levels of IL-1β,IL-18,IFI16,Caspase-1,GSDMD,IL-1βand IL-18 mRNA were increased(P<0.05).Yiaikang capsule could increase the number of CD4^(+)T lymphocytes and the ratio of CD4^(+)/CD8^(+)T lymphocytes,decrease the ra⁃tio of CD8^(+)/CD3^(+)T lymphocytes(P<0.05),increase the frequency of DC and decrease the level of DC necrosis(P<0.05)in HIV/AIDS patients with lung and spleen qi deficiency syndrome;the expression of IL-1β,IL-18,IFI16,Caspase-1,GSDMD,IL-1β,IL-18 mRNA were decreased(P<0.05).Conclusion The immune function of HIV/AIDS patients with syndrome of qi deficiency of lung and spleen is decreased,the frequency of DC is decreased,and the levels of relevant molecular genes and
