摘 要:目的:探讨新建自体动静脉内瘘早期维护对维持性透析患者昼夜血压节律和睡眠结构的作用。方法:选取2022年1月至2024年5月广州市第一人民医院收治的维持性透析患者60例作为研究对象,按照随机数字表法分为对照组和观察组,每组30例。对照组采用新建自体动静脉内瘘常规维护,观察组应用新建自体动静脉内瘘早期维护。观察2组患者昼夜血压节律以及睡眠结构的变化。结果:观察组全天收缩压、全天舒张压、日间收缩压、日间舒张压、夜间收缩压、夜间舒张压水平均低于对照组,且夜间△SBP%以及夜间△DBP%降低,差异有统计学意义(P<0.05)。观察组快速转动睡眠时间比例以及深睡眠比例大于对照组,且浅睡眠时间小于对照组,差异有统计学意义(P<0.05)。结论:新建自体静脉内瘘的持续性透析患者采取早期维护有利于稳定患者昼夜血压节律变化和睡眠结构,帮助患者有效进行血液透析。Objective:To investigate the effect of early maintenance of new autologous arteriovenous fistula on the circadian blood pressure rhythm and sleep structure of maintenance dialysis patients.Methods:A total of 60 patients with maintenance dialysis who received treatment from January 2022 to May 2024 were randomly selected for the study.The subjects were divided into two groups according to the random number table method,of which 30 patients were the control group with routine maintenance of autologous arteriovenous fistula and the other 30 patients were the experimental group with early maintenance of newly autologous arteriovenous fistula.The changes of circadian blood pressure rhythm and sleep structure-in the two groups were observed.Results:The 24-hour systolic blood pressure,24-hour diastolic blood pressure,daytime-systolic blood pressure,daytime diastolic blood pressure,nighttime systolic blood pressure and nighttime diastolic blood pressure levels in the experimental group were lower than those in the control group.Additionally,the nighttime△SBP%and nighttime△DBP%were decreased,and there were statistically significant differences between the two groups(P<0.05).The proportion of-rapid eye movement sleep time and deep sleep time in the experimental group was higher than that in the control group,and the light sleep time was lower than that in the control group,with statistical differences(P<0.05).Conclusion:Early maintenance is beneficial to the stability of circadian blood pressure rhythm and sleep structure of patients with newly autologous venous fistula in continuous dialysis,and helps patients to effectively perform hemodialysis.
