音乐电针调控阿尔茨海默病睡眠障碍30例临床观察

摘　　要：目的:观察音乐电针对阿尔茨海默病患者睡眠质量和认知水平的临床疗效。方法:选取2021年4月至2023年10月重庆市中医院睡眠门诊收治的阿尔茨海默病睡眠障碍患者108例作为研究对象,最终符合标准88例,按照随机数字表法分为音乐电针观察组(n=30)、脉冲电针对照组(n=29)和药物对照组(n=29)。治疗8周后,观察3组患者干预前后认知和睡眠评估,包括简易精神状态检查表(MMSE)、日常生活活动能力评定(ADL)、FOM、DS、匹兹堡睡眠质量指数(PSQI)评分,进行安全性评价。结果:3组患者治疗后PSQI评分均下降,差异有统计学意义(P<0.05),组间对比药物对照组睡眠改善最佳,差异有统计学意义(P<0.01),音乐电针观察组和脉冲电针对照组在改善日间功能方面差异显著,差异有统计学意义(P<0.01);音乐电针观察组和脉冲电针对照组认知评估(MMSE、ADL、FOM、DS评分)较药物对照组改善显著,差异有统计学意义(P<0.01),组间比较音乐电针组疗效最佳,差异有统计学意义(P<0.05)。结论:药物治疗阿尔茨海默病睡眠障碍虽然具有见效快、作用强的特点,但可加速患者认知与行为能力恶化,引起神经内分泌系统功能失调,日间功能受损,不良反应明显。音乐电针疗法可以安全有效改善阿尔茨海默病患者睡眠质量和认知水平,提升日常生活能力,值得临床应用。Objective:Observation of the clinical efficacy of music electroacupuncture on sleep quality and cognitive level in patients with Alzheimer′s disease.Methods:A total of 108 patients with Alzheimer′s disease and sleep disorders treated at the Sleep Clinic of Chongqing Hospital of Traditional Chinese Medicine from April 2021 to October 2023 were selected as the study subjects.Ultimately,88 cases met the criteria and were divided into three groups using a random number table:the music electroacupuncture treatment group(n=30),the pulse electroacupuncture control group(n=29),and the drug control group(n=29).After 8 weeks of treatment,the cognitive and sleep assessment including Mini-Mental State Examination(MMSE),Activities of Daily Living(ADL),FOM,DS,Pittsburgh Sleep Quality Index(PSQI)were observed before and after intervention,and the safety was evaluated.Results:The PSQI scores of all the three groups decreased after treatment(p<0.05).Intergroup comparisons showed that the drug control group had the best improvement in sleep,with a statistically significant difference(P<0.01).there was significant difference in improving daytime function between music electroacupuncture group and pulse electroacupuncture control group(p<0.01).The scores of MMSE,ADL,FOM and DS in the music electroacupuncture treatment group and the pulse electroacupuncture control group were significantly improved as compared with those in the drug control group(p<0.01),and the music electroacupuncture group had the best therapeutic effect(p<0.05).Conclusion:Although the drug therapy for sleep disorders in Alzheimer′s disease has the characteristics of rapid onset and strong action,it-may accelerate the deterioration of cognition and capacity,and cause the endocrine system dysfunction and the impairment of daytime function,and lead to obvious adverse reactions.Music electro-acupuncture therapy can safely and effectively improve the sleep quality and cognitive level of patients with Alzheimer′s disease,and enhance the ability of daily life,wh
