摘  要:目的:研究颅脑计算机体层成像(CT)扫描参数与学习记忆改变之间的关系。方法:选取2021年3月至2023年12月山东省德州市陵城区中医院影像中心收治的失眠症患者86例作为观察组,并选取同期体检的正常人86例作为对照组。应用CT灌注显像(CTP)测量脑血流量,中文版记忆症状评估量表(MSAS)评价人口学习记忆能力,采用多因素Logistic回归分析探究睡眠障碍的影响因素。结果:观察组外显学习记忆、内隐学习记忆、日常学习记忆、脑血容量和血流量均显著低于对照组,达峰时间和平均通过时间显著高于对照组,差异均有统计学意义(均P<0.05)。多因素Logistic回归分析结果发现,日常学习记忆评分(OR=4.030)、脑血容量(OR=8.490)、血流量(OR=5.094)等是导致睡眠障碍的影响因素,差异有统计学意义(P<0.05)。结论:睡眠障碍患者颅脑CT灌注成像指标有不同程度的改变,同时伴有学习记忆功能减退。Objective:To investigate the relationship between cranial computed tomography(CT)scan parameters and changes in learning and memory functions.Methods:A total of 86 patients with insomnia admitted to the Imaging Center of Lingcheng District Traditional Chinese Medicine Hospital in Dezhou City,Shandong Province from March 2021 to December 2023 were selected as the observation group,and 86 healthy individuals who underwent physical examinations during the same period were selected as the control group.CT perfusion imaging(CTP)was used to measure cerebral blood flow,and the Chinese version of the Memory Symptom Assessment Scale(MSAS)was used to evaluate the learning and memory abilities of the population.Multivariate logistic regression analysis was conducted to explore the influencing factors of sleep disorders.Results:The observation group had significantly lower levels of explicit learning and memory,implicit learning and memory,daily learning and memory,cerebral blood volume,and blood flow compared to the control group.The peak time and mean transit time were significantly higher than those of the control group,and the differences between the two groups were statistically significant(P<0.05).The results of multiple logistic regression analysis showed that daily learning and memory scores(OR=4.030),cerebral blood volume(OR=8.490),and blood flow(OR=5.094)were the influencing factors of sleep disorders,and the differences were statistically significant(P<0.05).Conclusion:Patients with sleep disorders exhibit varying degrees of changes in cerebral CT perfusion imaging indicators,accompanied by decreased learning and memory function.
