围术期快速康复干预模式对单侧人工膝关节置换术患者睡眠质量的影响

摘　　要：目的:分析在单侧人工膝关节置换术围手术期护理中采取快速康复干预模式的应用价值及对患者睡眠质量的影响。方法:选取2022年1月至2023年12月福州市第二总医院骨科收治的膝关节损伤行单侧人工膝关节置换术患者170例作为研究对象,按照随机数字表法分为对照组和观察组,每组85例。采用医院自制的专业问卷调查患者的护理满意度,采用视觉模拟评分(VAS)比较2组患者的疼痛改善效果,采用膝关节功能评分(KSS)量表比较2组患者治疗前后的膝关节功能状况,采用阿森斯失眠量表(AIS)比较2组患者睡眠障碍的改善效果,采用匹兹堡睡眠指数量表(PSQI)比较2组患者干预前后睡眠质量的改变情况。结果:观察组的总满意度显著高于对照组;干预后,观察组的VAS评分、PSQI评分、AIS评分均显著低于对照组;观察组膝关节功能评分KSS评分显著高于对照组,差异均有统计学意义(均P<0.05)。结论:在单侧人工膝关节置换术围术期采取快速康复干预可显著提高护理效果,改善患者的睡眠质量,较常规围术期护理更能缓解疼痛症状,促进膝关节功能的改善,提高护理满意度,可为临床提供有效借鉴。Objective:To analyze the application value of the enhanced recovery intervention mode in perioperative nursing of unilateral artificial knee replacement surgery and its impact on patients′sleep quality.Methods:A total of 170 patients with knee joint injuries undergoing unilateral artificial knee replacement surgery admitted to the Orthopedics Department of Fuzhou Second General Hospital from January 2022 to December 2023 were selected as the study subjects.They were randomly divided into a control group and an observation group using a random number table method,with 85 patients in each group.A self-designed hospital questionnaire was used to assess patient satisfaction with nursing care.The Visual Analog Scale(VAS)was used to compare the pain improvement effects between the two groups.The Knee Society Score(KSS)scale was used to compare the knee function status of the two groups before and after treatment.The Athens Insomnia Scale(AIS)was used to compare the improvement in sleep disorders between the two groups.The Pittsburgh Sleep Quality Index(PSQI)was used to compare changes in sleep quality before and after the intervention in both groups.Results:The overall satisfaction of the observation group was significantly higher than that of the control group;After intervention,the VAS score,PSQI score,and AIS score of the observation group were significantly lower than those of the control group.The knee joint function score KSS score of the observation group was significantly higher than that of the control group,and the differences between the two groups were statistically significant(both P<0.05).Conclusion:Implementing enhanced recovery intervention during the perioperative period of unilateral artificial knee replacement surgery can significantly improve nursing effectiveness,improve patients′sleep quality,alleviate pain symptoms more effectively than conventional perioperative nursing,promote knee joint function improvement,and increase nursing satisfaction.This approach provides valuable clinical referenc
