益心通络利水方治疗慢性心力衰竭临床观察

摘　　要：目的:观察益心通络利水方治疗慢性心力衰竭的临床疗效。方法:将60例慢性心力衰竭患者随机分为两组各30例。对照组予血管紧张素转换酶抑制剂、β受体阻滞剂等常规治疗,治疗组在对照组基础上联合益心通络利水方治疗。疗程均为4周。比较两组患者的临床疗效及左心房内径(LAD)、左心室内径(LVEDD)、左心室射血分数(LVEF)、N末端B型钠尿肽前体(NT-proBNP),左室整体纵向应变(GLS)、圆周应变(GCS)情况。结果:治疗组总有效率90.00%,对照组总有效率83.33%,两组比较差异有统计学意义(P﹤0.05)。治疗后,两组LAD、LVEDD、NT-proBNP水平均较治疗前降低(P﹤0.05),且治疗组低于对照组(P﹤0.05);两组LVEF水平均较治疗前升高(P﹤0.05),且治疗组高于对照组(P﹤0.05)。治疗后两组GLS、GCS水平均较治疗前降低(P﹤0.05),且治疗组低于对照组(P﹤0.05)。结论:益心通络利水方能够提高慢性心力衰竭患者的心功能,延缓心肌重构。Objective:To observe the clinical efficacy of Yixin Tongluo Lishui formula on chronic heart failure.Methods:60 cases of patients with chronic heart failure were randomly divided into two groups,30 cases in each group.The control group was treated by routine therapeutic regimen such as angiotensin converting enzyme inhibitor andβ-receptor blocker etc.while the treatment group was treated plus with Yixin Tongluo Lishui formula on the basis of control group.The course of treatment was 4 weeks.Clinical efficacy,left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),N-terminal pro-B-type natriuretic peptide(NT-proBNP),left ventricle global longitudinal strain(GLS)and left ventricle global circumferential strain(GCS)of patients were compared between two groups.Results:The total effective rate was 90.00%in the treatment group and 83.33%in the control group respectively,and the comparative difference between two groups was statistically significant(P﹤0.05).In both groups after treatment,the levels of LAD,LVEDD and NT-proBNP were lower than pre-treatment(P﹤0.05),and the treatment group was lower than control group(P﹤0.05);LVEF level was higher than pre-treatment(P﹤0.05),and the treatment group was higher than control group(P﹤0.05);the levels of GLS and GCS were lower than pretreatment(P﹤0.05),and the treatment group was lower than control group(P﹤0.05).Conclusion:Yixin Tongluo Lishui formula can improve the cardiac function of patients with chronic heart failure,and delay myocardial remodeling.
