祛湿化瘀汤联合非布司他片治疗湿浊瘀阻型尿酸性肾病临床观察

摘　　要：目的:观察祛湿化瘀汤联合非布司他片治疗湿浊瘀阻型尿酸性肾病的临床疗效。方法:将60例符合纳入标准患者按照随机数字表法分为两组各30例。对照组予非布司他片治疗,治疗组在对照组基础上予中药祛湿化瘀汤治疗,两组疗程均为12周。比较两组治疗前后血尿酸、血肌酐、尿素氮、肾小球滤过率、24小时尿蛋白定量、尿β2-微球蛋白等指标水平及中医证候评分。结果:治疗后,治疗组总有效率为93.33%,对照组为73.33%,组间比较差异有统计学意义(P<0.05);治疗组中医证候评分、血尿酸、血肌酐、尿素氮以及24小时尿蛋白定量、尿β2-微球蛋白水平均低于对照组(P<0.05),eGFR高于对照组(P<0.05)。结论:祛湿化瘀汤联合非布司他片治疗湿浊瘀阻型尿酸性肾病疗效较好,能够改善患者肾功能,降低24 h Upro、β2-MG水平,缓解临床症状。Objective:To observe the clinical efficacy of Qushi Huayu decoction and febuxostat tablets on uric acid nephropathy with syndrome of retention of turbid damp and blood stasis.Methods:60 cases of eligible patients with uric acid nephropathy were randomly divided into two groups,30 cases in each group.The control group was treated with febuxostat tablets,while the treatment group was treated plus with Qushi Huayu decoction of TCM on the basis of control group.The course of treatment was 12 weeks for both groups.The changes of following indexes were compared before and after treatment,such as serum uric acid,serum creatinine,urea nitrogen,glomerular filtration rate,24 h Upro,urineβ2-microglobulin and TCM syndrome score,etc.Results:After treatment,the total effective rate was 93.33%in the treatment group and 73.33%in the control group respectively,and the comparative difference between two groups was statistically significant(P﹤0.05);the levels of TCM syndrome score,serum uric acid,serum creatinine,urea nitrogen,24 h Upro and urineβ2-microglobulin of treatment group were lower than those of control group(P﹤0.05),while eGFR of treatment group was higher than that of control group(P﹤0.05).Conclusion:The clinical efficacy of Qushi Huayu decoction and febuxostat tablets on uric acid nephropathy with syndrome of retention of turbid damp and blood stasis is better,which can improve patients'renal function,reduce the levels of 24 h Upro andβ2-MG,and relieve clinical symptoms.
