羌防灵仙胜湿汤配合中药熏蒸法治疗类风湿关节炎临床观察

摘　　要：目的:观察羌防灵仙胜湿汤配合中药熏蒸法治疗类风湿关节炎的临床疗效。方法:选风寒痹阻型类风湿关节炎患者62例,随机分为两组各31例。对照组予尪痹片治疗,治疗组予羌防灵仙胜湿汤配合中药熏蒸法治疗。连续治疗2个月后,比较两组的临床疗效、中医证候评分、28个关节疾病活动度评分(DAS28)、VAS疼痛评分(VAS)以及C反应蛋白(CRP)、红细胞沉降率(ESR)、类风湿因子(RF)等指标水平。结果:治疗组总有效率(93.5%)高于对照组(67.7%),两组比较差异具有统计学意义(P﹤0.05)。治疗后,治疗组中医证候积分的改善效果优于对照组(P﹤0.05);两组DAS28、VAS、RF、CRP、ESR水平均降低(P﹤0.05),且治疗组明显低于对照组(P﹤0.05)。结论:羌防灵仙胜湿汤配合中药熏蒸法治疗风寒痹阻型类风湿关节炎疗效明显,能够有效缓解类风湿关节炎患者晨僵以及关节疼痛等症状,改善其中医证候及理化指标。Objective:To observe the clinical efficacy of Qiangfang Lingxian Shengshi decoction and fumigation of TCM on rheumatoid arthritis.Methods:62 cases of patients with rheumatoid arthritis with syndrome of wind-cold obstruction were selected and randomly divided into two groups,31 cases in each group.The control group was treated with Wangbi tablets,while the treatment group was treated plus with Qiangfang Lingxian Shengshi decoction and fumigation of TCM.The following indexes were compared between two groups after two months of continuous treatment,such as clinical efficacy,TCM syndrome score,disease activity score for 28 joints(DAS28),VAS pain score(VAS),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),rheumatoid factor(RF),etc.Results:The total effective rate of treatment group was 93.5%,which was higher than 67.7%of control group,and the comparative difference between two groups was statistically significant(P﹤0.05).After treatment,the improving effect of TCM syndrome score of treatment group was better than that of control group(P﹤0.05);the levels of DAS28,VAS,RF,CRP and ESR were all decreased in two groups(P﹤0.05),and the treatment group was significantly lower than control group(P﹤0.05).Conclusion:The clinical efficacy of Qiangfang Lingxian Shengshi decoction and fumigation of TCM on rheumatoid arthritis with syndrome of wind-cold obstruction is significant,which can effectively relieve the symptoms of patients with rheumatoid arthritis,such as morning stiffness and joint pain,etc.and improve TCM syndromes and physicochemical indexes.
