温针灸联合三阶梯止痛法治疗肿瘤晚期安宁疗护患者癌性疼痛临床观察

摘　　要：目的:观察温针灸联合三阶梯止痛法治疗肿瘤晚期安宁疗护患者癌痛的临床疗效。方法:将符合纳入标准的60例确诊为肿瘤晚期伴有癌性疼痛并接受安宁疗护的患者随机分为两组各30例。对照组患者根据WHO推荐的三阶梯止痛法给予阿片类药物止痛,治疗组在对照组基础上联合温针灸疗法,两组患者均治疗2周。观察两组患者治疗前后疼痛评分(NRS)、临床疗效、止痛药物用量、焦虑评分(GAD-7)、抑郁评分(PHQ-9)以及不良反应发生率。结果:治疗组总有效率90.00%,明显高于对照组的56.67%,差异具有统计学意义(P﹤0.05);治疗后治疗组NRS评分减低程度优于对照组(P﹤0.05);治疗组阿片类药物用量明显少于对照组(P﹤0.05),药物不良反应较对照组发生率更低(P﹤0.05);治疗后,治疗组患者焦虑评分(GAD-7)、抑郁评分(PHQ-9)较对照组降低更明显(P﹤0.05)。结论:温针灸联合三阶梯止痛法可有效减轻肿瘤晚期安宁疗护患者的癌性疼痛,减少止痛药物用量及药物不良反应,改善焦虑、抑郁状态,疗效较好。Objective:To observe the clinical efficacy of warming needle moxibustion and tri-step analgesia on cancer pain of palliatively treated and nursed patients with advanced tumor.Methods:60 cases of eligible patients,who were diagnosed as advanced tumor with cancer pain and received palliative treatment and nursing,were randomly divided into two groups,30 cases in each group.The control group was treated with opioid drugs for analgesia according to the tri-step analgesia recommended by WHO,while the treatment group was treated plus with warming needle moxibustion on the basis of control group.Both groups were treated for 2 weeks.Pain score(NRS),clinical efficacy,dosage of analgesic drugs,anxiety score(GAD-7),depression score(PHQ-9)and incidence rate of adverse reactions of patients were observed in two groups before and after treatment.Results:The total effective rate of treatment group was 90.00%,which was significantly higher than 56.67%of control group,and the difference was statistically significant(P﹤0.05).In the treatment group after treatment,NRS score was lower than that of control group(P﹤0.05);the dosage of opioids drugs was significantly less than that of control group(P﹤0.05),and the incidence rate of adverse drug reactions was lower than that of control group(P﹤0.05);the anxiety score(GAD-7)and depression score(PHQ-9)were significantly lower than those of control group(P﹤0.05).Conclusion:Warming needle moxibustion and tri-step analgesia can effectively relieve cancer pain of palliatively treated and nursed patients with advanced tumor,reduce the dosage of analgesic drugs and adverse drug reactions,and improve the anxious and depressive states,which has better clinical efficacy.
