基于“毛脉合精”理论探讨慢性肺源性心脏病缓解期的诊疗

摘　　要：慢性肺源性心脏病(chronic pulmonary heart disease,CPHD)是多种肺系疾病发展到终末期的结果,病机虚实夹杂,病程迁延难愈,病情复杂多变。缓解期的治疗决定疾病的转归预后,是治疗此病的关键。“毛脉合精”理论出自《黄帝内经》,是指气血相合。肺主气,心主血,生理情况下气血相关、心肺互用;病理情况下毛脉失和,气血不畅,正气亏虚,病情渐重。日久致心肺重构,表现出一系列心肺功能失常的症状。基于CPHD缓解期核心病机,治宜扶正化痰祛瘀。补气养血、益气滋阴、培元固本以扶正;化痰通络、活血化瘀以祛邪。如此,通调毛脉,则气血和,心肺畅,病可安。Chronic pulmonary heart disease(CPHD)is the result of the development of a variety of pulmonary diseases to the end-stage stage,with a mixture of deficiency and excess,prolonged course,and complex and changeable conditions.The treatment in remission determines the prognosis of the disease and is the key to the treatment of the disease.The theory of“integration of essence from the skin-hair and the vessels”comes from Inner Canon of Huangdi,which refers to the combination of Qi and blood.Lung governs Qi and heart governs blood.The correlation of Qi and blood under physiological conditions is the mutual use of heart and lung.Under the pathological conditions,the skin-hair and the vessels are disharmonious,Qi and blood are not smooth,the righteous Qi is deficient,and the disease is gradually serious.Over time,it causes cardiopulmonary remodeling,showing a series of symptoms of cardiopulmonary dysfunction.Based on the core pathogenesis of CPHD in remission period,it is advisable to treat it with strengthening body resistance,resolving phlegm and removing blood stasis.The treatment should supplement Qi and nourish blood,benefit Qi and nourish Yin,and cultivate righteous Qi to strengthen the foundation;resolve phlegm and dredge collaterals,promote blood circulation and dispel blood stasis to dispel evils.In this way,if the hair veins are regulated,Qi and blood will be harmonious,the heart and lung will be normal,and the disease can be relieved.
