网格化管理模式在农村高血压病患者管理中的应用效果与评价

摘　　要：目的:评价网格化管理模式在农村高血压病患者管理中的应用效果。方法:采用便利抽样法,对2021年6月—2021年12月网格化管理实施前的广西上林G村115例高血压病患者(对照组)实施常规管理模式,对2022年3月—2022年9月网格化管理实施后的广西上林G村102例高血压病患者(观察组)实施联动上级医疗机构(后盾单位)、乡镇政府、卫生院(室)、村委会等构建农村慢性病网格化管理模式。对比两组患者的服药依从性、生活质量以及管理后1个月、3个月、6个月的血压管理成效。结果:实施网格化管理模式后,观察组患者的服药依从性、生活质量、血压管理成效均高于对照组,差异有统计学意义(P<0.05)。结论:网格化管理模式用于农村高血压病患者,能有效提高患者依从性,控制患者病情,提升生活质量,同时能有效规避“因病致贫、因病返贫”的风险,推动健康乡村,助力乡村振兴。Objective:To evaluate the application effect of grid-based management model in the management of rural hypertensive patients.Methods:Convenience sampling method was used,and before the implementation of grid-based management was taken as the control group,and after the implementation of grid-based management was taken as the observation group.Before the implementation of grid-based management(June 2021 to December 2021),115 hypertensive patients in G village of Shanglin in Guangxi were selected as the study subjects,and the conventional management mode was adopted;after the implementation of grid-based management(March 2022 to September 2022),102 hypertensive patients in G village of Shanglin in Guangxi were selected as the study subjects,and the linkage of higher-level healthcare institutions(back-up units),township governments,health hospitals(rooms),and village committees to construct a grid-based management model for chronic diseases in rural areas.Compare the medication adherence and quality of life of patients in the control group and the observation group,as well as the effectiveness of blood pressure management of patients after 1 month,3 months and 6 months of management.Results:After the implementation of the grid-based management model,the medication compliance,quality of life,and blood pressure management effectiveness of patients in the observation group were significantly higher than those in the control group(P<0.05).Conclusion:The grid-based management model is suitable for the management of rural hypertensive patients,can effectively improve patient compliance,thus controlling the patient's condition and further improving the patient's quality of life,and at the same time,can effectively avoid the risk of`poverty due to illness and return to poverty due to illness',promote a healthy countryside,and contribute to the revitalization of the countryside.
