摘 要:目的:检测轮状病毒(RV)感染性腹泻患儿外周血Toll样受体(TLRs)表达水平及肠道菌群分布,分析其临床意义。方法:选择2023年6月至2024年6月我院收治的80例RV感染性腹泻患儿,检测患儿入院第1天(治疗前)、治疗第7天(治疗后)血液TLRs mRNA水平和肠道菌群水平,比较不同病程(≥7 d、<7 d)和Vesikari评分(≥9分、<9分)RV感染性腹泻患儿之间的差异。结果:1)治疗前血液TLRs mRNA水平,病程≥7 d RV腹泻患儿TLR2 mRNA、TLR3 mRNA、TLR4 mRNA、TLR7 mRNA、TLR8 mRNA、TLR9 mRNA水平均高于病程<7d者,Vesikari评分≥9分者上述TLRs mRNA水平亦明显高于Vesikari评分<9分者,P<0.05;TLR1 mRNA、TLR5 mRNA、TLR6 mRNA、TLR10 mRNA水平,不同病程及不同Vesikari评分组无明显差异,P>0.05。2)治疗前肠道菌群,病程≥7 d RV腹泻患儿肠道乳酸杆菌、双歧杆菌、拟杆菌属和梭菌属水平明显低于病程<7 d者,Vesikari评分≥9分者上述菌群水平亦明显低于Vesikari评分<9分者,P<0.05。3)治疗后RV腹泻患儿肠道乳酸杆菌、双歧杆菌、拟杆菌属和梭菌属均较治疗前明显升高,TLR2 mRNA、TLR3 mRNA、TLR4 mRNA、TLR7 mRNA、TLR8 mRNA、TLR9 mRNA明显降低,P<0.05;其他TLRs mRNA无明显差异,P>0.05。结论:RV感染性腹泻患儿外周血TLRs表达异常,肠道菌群改变,且它们与RV感染病程及病情严重程度相关,其治疗或可通过调整外周血TLRs表达及菌群分布实现。Objective To detect the expression levels of Toll-like receptors(TLRs)in peripheral blood and the distribution of intestinal flora in children with RV infectious diarrhea,and to analyze their clinical significance.Methods Total 80 children with RV infectious diarrhea admitted to our hospital from June 2023 to June 2024 were selected.The levels of blood TLRs mRNA and intestinal flora were detected on the lst day of admission(before treatment)and the 7th day of treatment(after treatment).The differences of children with RV infectious diarrhea of different course of disease(≥7 days,<7 days)and Vesikari scores(≥9 points,<9 points)were compared.Results1)The blood mRNA levels of TLRs before treatment,the mRNA levels of TLR2,TLR3,TLR4,TLR7,TLR8 and TLR9 in children with RV infectious diarrhea with course of disease≥7 days were higher than those in children with course of disease<7 days.The mRNA levels of TLRs in patients with Vesikari scores≥9 were significantly higher than those in patients with Vesikari scores<9(P<0.05).There were no significant differences in the mRNA levels of TLR1,TLR5,TLR6,and TLR1o between groups with different disease durations or Vesikari scores(P>0.05).2)Before treatment,the children with RV infectious diarrhea with course of disease≥7 days had significantly lower levels of Lactobacillus,Bifidobacterium,Bacteroides,and Clostridium than those with course of disease<7 days(P<0.05),and the children with Vesikari scores≥9 had significantly lower levels of the above microbes than those with Vesikari scores<9(P<0.05).3)After treatment,the levels of Lactobacillus,Bifidobacterium,Bacteroides and Clostridium were significantly higher than those before treatment,and the mRNA levels of TLR2,TLR3,TLR4,TLR7,TLR8 and TLR9 were significantly lower than those before treatment(P<0.05).There was no significant difference in other TLRs mRNA(P>0.05).Conclusion There are abnormal expressions of TLRs in peripheral blood and changes in intestinal flora in children with RV infectious diarrhea,which are re
