支撑喉镜下CO_(2)激光切除术治疗双侧广基型声带息肉的效果

摘　　要：目的:分析支撑喉镜下CO_(2)激光切除术治疗双侧广基型声带息肉的临床效果。方法:选取2022年7月—2024年5月北京怀柔医院耳鼻喉科收治的60例确诊为双侧广基型声带息肉的患者。通过病历号单双数法将患者随机分为试验组(n=30)、对照组(n=30)。对照组患者行常规声带息肉切除术,试验组行支撑喉镜下CO_(2)激光切除术。比较两组临床有效率、复发率、术后首次发声时间、嗓音恢复时间、住院时间、嗓音障碍指数(VHI)、嗓音功能指标、并发症发生率。结果:试验组临床有效率较对照组高,复发率较对照组低,差异有统计学意义(P<0.05)。试验组术后首次发声时间、嗓音恢复时间、住院时间较对照组短,差异有统计学意义(P<0.05)。术后,两组VHI、嗓音功能指标均改善,且试验组术后最长发声时间(MPT)较对照组长,VHI、基频微扰(Jitter)、振幅微扰(Shimmer)较对照组低,差异有统计学意义(P<0.05)。试验组声带粘连发生率为6.7%(2/30),对照组为13.3%(4/30),两组比较差异无统计学意义(χ^(2)=0.740,P=0.389)。结论:双侧广基型声带息肉患者采用支撑喉镜下CO_(2)激光切除术治疗效果显著,术后复发率较低,术后恢复时间短,嗓音功能恢复良好,不会导致声带粘连发生率升高。Objective:To evaluate the clinical effect of CO_(2) laser resection under supported laryngoscope in the treatment of bilateral wide basal polyps of vocal cords.Method:A total of 60 patients diagnosed with bilateral wide basal polyps of vocal cords were selected in Department of Otolaryngology of Beijing Huairou Hospital from July 2022 to May 2024.The patients were randomly assigned to the experimental group(n=30)and the control group(n=30)by using the odd-even number method of medical record number.The control group underwent routine vocal cord polypectomy,while the experimental group underwent CO_(2) laser resection under supported laryngoscope.The clinical response rate,recurrence rate,postoperative first voice time,voice recovery time,hospital stay,voice disturbance index(VHI),voice function index,and complication rate of the two groups were compared.Result:The clinical effective rate of experimental group was higher than that of control group,and the recurrence rate of experimental group was lower than that of control group,the differences were statistically significant(P<0.05).The first sound time,voice recovery time and hospitalization time were shorter in experimental group than those in control group,the differences were statistically significant(P<0.05).After surgery,the VHI and voice function indexes of both groups were improved,and the longest voice time(MPT)of experimental group was longer than that of control group,the VHI,Jitter and Shimmer of experimental group were lower than those of control group,the differences were statistically significance(P<0.05).The incidence of vocal cord adhesion was 6.7%(2/30)in experimental group and 13.3%(4/30)in control group,there was no significant difference between the two groups(χ^(2)=0.740,P=0.389).Conclusion:The effect of CO_(2) laser resection under supported laryngoscope on patients with bilateral wide basal polyps of vocal cords is significant,the postoperative recurrence rate is low,the postoperative recovery time is short,the voice function is recovered
